PRN Mobile Nurse Practitioner – West Virginia, South Region | $85/hr

Avail Health is hiring a Mobile Nurse Practitioner to support I-SNP with Medicare Advantage members in long-term care settings across West Virginia, South Region.

This is a 1099 PRN role with flexible scheduling and consistent patient volume.

What You’ll Do

• Conduct in-person chronic disease assessments in long-term care facilities
• Complete Health Risk Assessments (HRAs)
• Develop care plan recommendations (problem, goal, interventions)
• Partner with operations for scheduling and documentation
• Work independently in the field with strong clinical support

Compensation

• $85/hour (1099)
• Mileage reimbursement: $0.725/mile (over 30 miles/day, up to $200/day)
• Training rate: $50/hour

Schedule

• PRN (as needed)
• Typically 2–3 days/week based on your availability
• Weekdays only: Monday–Friday, 8am–5pm
• You set your availability; scheduling is coordinated accordingly

What You Need

• NP (FNP or Adult-Gerontology) – active West Virginia license
• 3+ years NP experience (geriatrics, primary care, or similar preferred)
• Comfortable working independently in a mobile setting
• Reliable transportation and willingness to travel locally
