Job Overview

Lead a dynamic team of leadership, case managers, family support specialists, and employment specialists, focused on compassionate and holistic community services for person identified as having a SPMI and need extra support in getting linked with all areas of services to help maximize and achieve overall life wellness. Increase relationships with community providers in Gloucester and Cape May County to ensure individuals are linked with services that they can feel identified/comfortable engaging with. Ensure full program regulatory and program processes while motivating a team. Participate in innovative initiatives designed to engage communities in care and treatment. Collaborate with Acenda team members to implement services for those populations identified needing support.

Program Info

Acute Services under the Director of Case Management & Community Support Services include targeted case management and community supports programs: ICMS Gloucester and Cape, Bridge, PATH Gloucester, SEP, and IFSS Gloucester and Cape May. Services provide intensive and integrated care for individuals with Serious and Persistent Mental Illness (SPMI) who are being discharged to the community from hospitals, screening, urgent care and other acute care providers to provide stabilization in the community. Intensive Family Support Services (IFSS) offer supportive care for families with loved ones with SPMI.

Key Responsibilities:

  • Provide supervision of staff and oversight of referrals/caseloads, as well as overall program and staff performance.
  • Monitor program budgets, adhere to agency and Medicaid/DMHAS state financial expectations, address billing issues, and insurance requirements.
  • Manage program reporting expectations and submit timely data of LOS/KPIs/QCMRs and additional reporting as required by agency, contract, and regulations.
  • Oversee program/staff meetings and adherence to state regulations.
  • Meet with regulatory and contractual partners, establish and maintain relationships with referral sources and acute care system provides.
  • Demonstrates understanding of state reporting requirements and appropriately tracks data and reports monthly/quarterly with the support of director.
  • Attends county and state meetings as required.
  • Identify and implement innovative program improvements and expansion opportunities to enhance program delivery.
  • Collaborate across agency divisions to meet program needs.
  • Develop program teams through training opportunities.
  • Engage in internal and external meetings, committees, and trainings.
  • Adhere to all program and state regulatory requirements as identified in the specific regulations and contracts in the role of director.
  • Performs other related duties as assigned.

Requirements:

  • Master’s Degree in social work, counseling, psychology or related field, from an accredited institution and possess one (1) year of relevant supervisory experience
  • At least 5 years of experience with serious mental illness population.
  • Experience with providing and managing targeted case management with adults.
  • Proven ability to manage successful fee for service program delivery and meet/exceed revenue expectations.
  • Proven ability to build strong relationships with stakeholders, community and referral sources and drive program referral volume.
  • Must have and maintain a valid driver's license with an acceptable driving record
  • Must be able to exercise independent thinking and good judgment under all circumstances.
  • Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.

Additional Information:

  • Base Pay: $80,000
  • Sign-on Bonus: $1000
  • Bilingual Rate: An additional $3,120 rate added to the base pay if Spanish proficient.
