View Point health is a community behavioral health center utilizing a team of psychiatrist, nurses, licensed counselors and social workers, as well as other professional to provide services to individuals who need treatment and support to cope with mental illness, substance abuse/use, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, ware veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support.


Under general supervision, plans, manages, and evaluates social services programs. Administers and coordinates the services and program activities in dependency, mental illness, youth development, or physical or developmental disability programs.


Duties & Responsibilities:

  • Provides leadership and guidance for teams of staff who provide groups and community activities for individuals with developmental disabilities in Day Services, Supported Employment, Family Support and Community Living Support programs.
  • Assists with the development of individualized service plans for each client by members of an interdisciplinary team.
  • Assures that plans are implemented and updated as required.
  • Reviews staff documentation for conformity to agency and payer requirements.
  • Appropriately interprets, implements, and enforces compliance with agency policies and procedures and safety regulations.
  • Assures that program participants receive the appropriate attention to their needs and service guidelines.
  • Recommend hires, trains, and evaluate performance of subordinate staff.
  • Utilizes coaching and involves subordinates in the identification and solution of program issues and the implementation of required program changes.
  • Adheres to licensure requirements and regulations as set forth by regulatory agencies
  • Participate actively in agency quality assurance activities including performance and outcomes review, case record review, stakeholder satisfaction review, incident review, and improvement planning. (DDP role)
  • Oversee services and support provided to participants for general guidance to the provider agency in areas of compliance and quality improvement.
  • Assuring that the support provided is within the scope of the agency’s service enrollment and experience to ensure effective delivery.
  • Assuring that the services address the participant’s needs and adhere to the application of person-centered values, choice and participant’s rights.
  • Providing, arranging or overseeing curricula used in staff training and directed to service delivery in the context of the individual’s goal(s) and objectives.
  • Recommend other services needed or changes to the delivery model using a continuous quality improvement approach.
  • Providing consultation to the provider agency in ISP implementation strategies that are specific, measurable, achievable, relevant, realistic and time limited in order to meet the needs and personal goal(s) of the participant.
  • Assess areas of risks either individually or overall risks to people supported through agency practice, policy or lack of policy or procedures/protocols. Providing risk mitigation strategies to the provider agency.
  • Reviewing functional assessments are in place to support formulation of the participant’s plan for delivery of all waiver services that include:
o The Health Risk Screening Tool.
o The Supports Intensity Scale.
o Functional Behavioral Evaluation.
o Others (E.g., Nursing, OT, PT etc.) as needed or required.
  • Oversee high intensity services if applicable that address health and safety risks for the participants that include:
o The implementation and effectiveness of Behavior Support Plans.
o The implementation and effectiveness of the Participant’s Crisis Plan; and
o Identifying ongoing supports as needed (medical and /or behavioral) in collaboration with agency
o staff, staff of other agencies providing support to the participants mutually served or other
o members of the healthcare team.
  • Other related duties as assigned.


Minimum Qualifications:

Bachelor's degree in a related field from an accredited college or university and two (2) years of job-related experience; or two (2) years of experience at the lower-level Social Services Program Coordinator 1 (SSP030) or position equivalent.

Note: Some positions require eligibility for licensure.


Preferred Qualifications:

  • Master’s degree in a related field.
  • At least 3 years of experience working with individuals with Developmental Disabilities.
  • Developmental Disabilities Professional Certification Eligible.


Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must have valid Georgia driver’s license and Motor Vehicle Record in accordance with company policy.
  • Candidates for selection must pass a criminal background check (including fingerprinting).
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Any combination of training and experience which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.

Note: Some positions may require a valid driver's license.
