About the Role

Balanced Wellbeing, LLC is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our growing behavioral health team providing psychiatric services in skilled nursing, long-term care, and assisted living facilities throughout the Jacksonville, Florida area.

This position focuses on psychiatric evaluation, medication management, and ongoing behavioral health care for adult and geriatric patients in facility-based settings. Providers work collaboratively with medical teams while maintaining autonomy in their clinical practice.

Compensation

• Base Salary: $125,000 – $135,000 annually
• Quarterly Performance Bonuses with strong earning potential

Key Responsibilities

• Conduct comprehensive psychiatric evaluations and mental health assessments
• Diagnose and treat psychiatric disorders in adult and geriatric populations
• Provide medication management and treatment planning
• Collaborate with facility staff, physicians, and care teams
• Maintain accurate and timely documentation in the electronic medical record
• Provide ongoing follow-up care and adjust treatment plans as needed

Qualifications

• Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
• Active Florida APRN license or ability to obtain
• Active DEA license (or ability to obtain)
• Active NPI number
• Experience in behavioral health, psychiatry, skilled nursing, or long-term care preferred
• Strong communication and clinical decision-making skills
