Full-Time Psychiatric Mental Health Nurse Practitioner (PMHNP) – Baton Rouge, LA
Homedica is seeking a compassionate and skilled Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide comprehensive psychiatric evaluation and medication management services across home-based, assisted living, skilled nursing, post-acute, and telehealth settings.
This is a full-time position offering a flexible schedule within a non-traditional care model, allowing you to practice with a high degree of autonomy while caring for medically complex and geriatric patients.
As part of an interdisciplinary team, you will serve as a behavioral health specialist, working alongside other providers to deliver integrated, patient-centered care.
Responsibilities:
- Conduct comprehensive psychiatric evaluations and ongoing medication management
- Develop and adjust individualized treatment plans for conditions such as depression, anxiety, and dementia-related behaviors
- Collaborate with other providers and interdisciplinary care teams
- Provide consultative support to caregivers, facility staff, and community partners
- Deliver care across in-home, facility-based, and telehealth environments
- Participate in after-hours or on-call rotation as needed
Qualifications:
- Active and unrestricted Louisiana Nurse Practitioner license
- PMHNP certification required
- Active DEA license
- Strong clinical judgment and ability to work independently across diverse care settings
Preferred:
- Experience managing behavioral health conditions in geriatric or medically complex populations
- Prior experience in facility-based, home-based, or post-acute care
- Familiarity with telehealth platforms and EMRs
- Dual certification (FNP/PMHNP) with willingness to support both roles
