The Role You Play

As a PRTF Case Manager, you will help consumers and families navigate the Psychiatric Residential Treatment Facility admission process, support coordination of care while the consumer is in treatment and assist with discharge planning to ensure continued connection to services and resources. This role works closely with consumers, families, treatment teams, schools, and community partners to support continuity of care.

Your Day-to-Day:

Coordinate PRTF admission and review processes by scheduling PAR and CBST meetings, gathering required information, and submitting state documentation within required timelines.
Support consumers and families before admission by providing outreach, referrals, resources, and ongoing follow-up while they wait for placement or if admission is denied.
Participate in treatment plan reviews, discharge calls, and ongoing care coordination with consumers, families, treatment teams, schools, and other approved supports.
Help develop post-discharge plans by coordinating follow-up appointments, connecting consumers to appropriate services, and supporting a smooth transition back into the community.
Advocate for client rights, dignity, confidentiality, and community integration while providing education and support related to mental health needs and available services.
Complete documentation on time, maintain clinical schedules, participate in supervision and staffing, and travel to community settings as needed to provide services.

Qualifications:

Required:

Bachelor’s degree in a human service field or equivalent work experience, or a combination of work experience in the human services field and education with one year of experience substituting for one year of education required.
Strong oral and written communication skills using the English language required.
A valid driver’s license and automobile liability insurance maintained in accordance with Pawnee policy and procedure is required.
Results of criminal and all other applicable background checks and drug screening must meet the approval of Pawnee Mental Health Services.

Preferred:

Knowledge of the needs and challenges of consumers who experience serious symptoms or mental illness and their families preferred.
Basic personal computer skills which enable the entry, retrieval and use of electronic data required; experience with Microsoft Office software preferred.
