A Quality Assurance (QA) Nurse in a
Skilled Nursing Facility (SNF) is a leadership role focused on maintaining clinical integrity, patient safety, and strict adherence to state and federal regulations. They bridge the gap between administrative goals and daily clinical practices to ensure high-quality resident outcomes.

QAPI Leadership: Direct and coordinate the facility’s Quality Assurance and Performance Improvement (QAPI) program to identify and resolve systemic issues.

Clinical Auditing: Conduct regular audits of medical records, patient charts, and nursing documentation to verify accuracy and compliance with Medicare and state guidelines.

Regulatory Readiness: Prepare the facility for annual surveys and inspections, acting as a liaison during regulatory visits.

Incident Analysis: Investigate adverse events (e.g., falls, medication errors), perform root cause analysis, and implement corrective action plans.

Staff Education: Develop and deliver training programs on clinical best practices, infection control, and new regulatory standards.

Infection Control & Safety: Often shares duties with the Infection Preventionist to monitor facility-acquired infections and maintain a safe environment.
Typical Requirements & Skills

Education: A valid state license as a Registered Nurse (RN) is standard, though some facilities accept Licensed Practical/Vocational Nurses (LPN/LVN) with significant experience.

Experience: Typically requires 2–5 years of experience in long-term care or skilled nursing.

Knowledge: Deep understanding of Federal Title 22 (in CA) or equivalent state regulations and CMS (Centers for Medicare & Medicaid Services) guidelines.

Analytical Skills: Proficiency in data analysis to track trends, quality indicators, and performance metrics.

Certifications: Optional but preferred certifications include CPHQ (Certified Professional in Healthcare Quality).
