QUALIFIED MENTAL HEALTH PROFESSIONAL

Responsibilities:
Qualified Mental health Professional- OTP
The Qualified Mental Health Professional will provide crisis intervention and clinical support to individuals involved in diversion and re-entry programs who are incarcerated or recently released. The primary goal is to stabilize mental health crises, assess client needs, and facilitate connections to appropriate community resources and treatment services. The position requires collaboration with correctional facilities, community partners, and healthcare providers to support successful reintegration and reduce recidivism. The Qualified Mental Health Professional (QP) will provide crisis response support, linkage to outside agencies to whom participants are referred, care coordination, and connection to Emergency Departments and the Behavioral Health Urgent Care Center. The Guilford County Sheriff’s Office Re-entry Program and Jail Diversion Program serves individuals who interface with the criminal justice system due to serious mental illness and substance use. The Guilford County Sheriff's Office, in collaboration with Foundations Health High Point will provide Medication Assisted Treatment (MAT) services for individuals detained in the Guilford County Detention Center who have a Substance Use Disorder (SUD).

Responsibilities:
  • Conduct timely, comprehensive mental health and substance abuse crisis assessments for individuals involved in diversion and re-entry programs.
  • Develop individualized care plans focusing on crisis management, mental health stabilization, and linkage to treatment.
  • Provide immediate crisis intervention and stabilization techniques to individuals experiencing acute mental health episodes.
  • Coordinate and facilitate referrals to community-based mental health and substance use treatment providers, housing agencies, social services, and other support systems
  • Monitor client progress and adjust service plans as necessary, ensuring continuity of care during and after release.
  • Collaborate closely with correctional staff, probation officers, social workers, and healthcare professionals to support integrated care.
  • Maintain accurate and timely documentation of all clinical interactions, assessments, treatment plans, and progress notes in compliance with regulatory and organizational standards.
  • Participate in multidisciplinary team meetings and case conferences to review client progress and coordinate care strategies.
  • Advocate for clients’ rights and access to services both within correctional settings and in the community.
  • Support program development initiatives aimed at enhancing crisis support and diversion services.

This position is supported by grant funding for an initial 18 month period, with the opportunity to extend for another 12 months. Following the grant period, there may be opportunities for the role to continue as part of ongoing operations, contingent on program sustainability and organizational needs.

Qualifications:
  • High School diploma/ GED required
  • Bachelor's degree preferred
  • A valid drivers’ license and proof of auto insurance is required for this position. Must also have a reliable means of transportation.
  • Minimum one year of full-time, post-graduate MH/DD/SAS experience with the population served or be a substance use professional with one full-time, post-degree supervised experience in alcohol and drug use counseling.
  • Prior experience working with justice-involved or formally incarcerated individuals.
