PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings.


New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens.

This is a grant-funded position ending March 31, 2027.


Program Description:

The Healthcare-Community Partnerships (HCP) Capacity Building team applies best practices and innovative methods from PHS’ experience in healthcare capacity building and quality improvement (QI) to the execution of scalable capacity building projects relevant to the latest healthcare reforms that attempt to improve health access and equity in New York. The team leads provider capacity building efforts for WholeYouNYC—including training, technical assistance, performance management, and QI—focused on strengthening providers’ implementation of SCN services and ensuring they are delivered in a high-quality, equitable, and accessible way.


Position Summary:

We seek an experienced, skilled and solutions-driven Quality Improvement (QI) Manager to lead and enhance WholeYouNYC’s provider-facing capacity building and quality improvement efforts, with an emphasis on healthcare providers. Reporting to the Director, HCP Capacity Building, the QI Manager will supervise a team of Practice Improvement (PI) Specialists, who are responsible for ensuring that WholeYouNYC providers successfully implement Social Care Network (SCN) services. The QI Manager will also serve as the internal expert on QI implementation and guide efforts to integrate quality improvement into WholeYouNYC’s performance management approach.


The QI Manager will play a pivotal role in managing and supporting WholeYouNYC’s provider-facing staff to strengthen provider performance, address barriers, and promote adoption of best practices that drive measurable impact. The effectiveness of this position will be directly tied to the performance of the PI Specialists they supervise—including their knowledge, skills, and ability to support providers—as well as the resulting performance and outcomes of providers across the network. The role calls for a collaborative, analytical leader with deep experience in QI, provider engagement, and implementing initiatives that support delivery of new services in complex health and social service settings.


Specifically, the QI Manager will:

  • Supervise a team of PI Specialists, providing guidance, coaching, support, and professional development to ensure effective provider engagement and SCN service delivery.
  • Collaborate with internal PHS teams to maintain up-to-date expertise on SCN services, workflows, implementation best practices, systems, requirements, policies, and procedures to guide effective provider support and QI strategies.
  • Advise on provider issues to identify root causes using data, dashboards, and analytic tools (e.g., PowerBI), elevate systemic barriers, and recommend actionable solutions.
  • Synthesize trends, challenges, and strengths across providers to inform network strategy and targeted training, technical assistance, and QI supports.
  • Collaborate with the TTA Manager to align provider-facing support across healthcare and community-based organizations.
  • Manage the implementation of performance management activities, including provider scorecards, performance metric analysis, and provider performance conversations and communications.
  • Manage the planning and execution of QI learning convenings and one-on-one coaching/TA to support providers’ adoption of best practices.
  • Develop QI tools, frameworks, and resources to the end of strengthening provider performance and adoption of best practices, adapting content as needed based on emergent issues and/or needs.
  • Serve as an internal expert on QI methodology and implementation strategies, ensuring provider-facing strategies and supports delivered by PI Specialists align with best practices.
  • Support Director to integrate QI into program strategy, performance management, and partner supports.


Qualifications and Experience:

  • Bachelor’s degree in a relevant field; Masters preferred.
  • 3-5 years of experience in a program/project management, managerial or supervisor role and/or equivalent.
  • Strong management and supervisory experience, including leading teams, solving complex programmatic challenges and supporting professional development.
  • Demonstrated expertise in QI and capacity building, preferably in healthcare settings.
  • Understanding of community, city and state health department systems and NYC-based provider networks. Prior experience in issues related to healthcare; social services; health equity, access and quality; population health (or some subset/combination of these) preferred.
  • Knowledge of HRSN screening, referral, and navigation processes preferred.
  • Experience analyzing and presenting data to groups and individuals, including use of dashboards and analytic tools.
  • Proven ability to work collaboratively with clinical providers, staff, and external partners.
  • Excellent organizational, time management, attention to detail, and oral and written communication skills.
