Position Overview

The Rapid Re-Housing (RRH) Case Manager is responsible for helping individuals and families quickly exit homelessness and transition into stable community housing through housing-focused case management, service coordination, landlord engagement, and stabilization support.

This role works closely with clients to identify barriers, secure housing opportunities, connect individuals to supportive services, and promote long-term housing retention and self-sufficiency. The RRH Case Manager plays a critical role in supporting clients as they transition from homelessness into stable housing while maintaining a client-centered, housing-first, and trauma-informed approach.

Productivity

· Complete monthly office and home visits with clients to monitor housing stability and identify barriers

· Identify and problem-solve housing concerns that may impact housing retention or successful program participation

· Coordinate with housing teams and service providers regarding clients requiring increased engagement or intervention

· Develop, implement, and maintain individualized service plans and housing stabilization plans that support clients in achieving and maintaining permanent housing.

· Complete annual re-certifications, reassessments, and all required program documentation within established timelines.

· Refer eligible clients to the CMH Housing Choice Voucher Program to support long-term housing stability.

· Assist clients in obtaining, organizing, and submitting all required documentation for housing and voucher programs.

· Scan and maintain required client documentation in VESTA and ensure screening packets are complete, accurate, and submitted by established deadlines.

· Begin exit planning at intake and continuously document both client and case manager actions taken to support long-term housing stability and successful program exit.

· Complete monthly home visits, office visits, and other required client contacts in accordance with program requirements.

· Connect clients with community resources including employment services, healthcare, behavioral health treatment, substance use treatment, transportation, benefits, and other supportive services.

· Document maintenance concerns reported by clients and conduct landlord follow-up as needed to support housing retention.

· Respond promptly and professionally to emails, phone calls, and communications from clients, staff, landlords, vendors, community partners, and referral agencies.

· Transport clients to appointments, housing-related meetings, inspections, lease signings, and other program-related activities as needed.

· Maintain accurate, timely, and compliant case notes, progress notes, assessments, and documentation within VESTA.

· Collaborate with landlords, housing providers, and community partners to identify and resolve barriers that may impact housing placement or retention.

Qualifications & Requirements

· Bachelor’s Degree preferred; equivalent education and experience may be considered

· Experience in ISP/case plan development and progress notes preferred

· Experience working with homelessness, housing services, behavioral health, or community-based programs preferred

· Knowledge of housing-first models, landlord engagement, and supportive services preferred

· Valid driver’s license with acceptable driving record required

· Must maintain access to privately owned vehicle during work hours

· Must be approved by Shelterhouse insurance provider for driving responsibilities

· Must be able to work flexible hours including weekends and holidays as needed

Skills & Abilities

· Knowledge of homelessness, housing-first principles, landlord engagement, and community resources

· Strong communication, customer service, and interpersonal skills

· Ability to work effectively in a high-energy and fast-paced environment

· Ability to work independently and manage competing priorities effectively

· Strong organizational and case management skills

· Ability to maintain professionalism, empathy, and confidentiality at all times

· Strong problem-solving and housing coordination skills

· Ability to build relationships with landlords, clients, and community partners effectively

Physical Requirements (% of time spent performing requirements)

· Frequent walking and movement throughout shelter facilities, housing sites, and community locations (approximately 70%)

· Prolonged sitting, documentation, and computer usage (approximately 30%)

· Ability to lift and carry up to 50 pounds as needed

· Ability to travel locally for home visits, appointments, landlord meetings, and housing coordination activities
