Overview
Join our dedicated team as a Reentry and Substance Use Disorder (SUD) Case Manager, where you will play a vital role in guiding individuals through their recovery journey. This dynamic position offers the opportunity to provide comprehensive case management and clinical support to clients battling addiction and co-occurring mental health conditions. You will coordinate care, conduct assessments, and deliver evidence-based interventions to empower clients toward lasting recovery. Responsibilities
- Conduct thorough intake assessments and diagnostic evaluations to determine client needs and develop personalized treatment plans in Pierce County Jail and outside.
- Provide individual and group counseling sessions utilizing therapeutic modalities such as cognitive behavioral therapy, motivational interviewing, and behavioral health techniques.
- Implement crisis intervention strategies to support clients experiencing acute mental health or substance use crises, ensuring safety and stabilization.
- Coordinate with healthcare providers, community agencies, and family members to facilitate integrated care plans that address medical, psychological, and social needs.
- Maintain accurate medical documentation in compliance with HIPAA regulations, including progress notes, treatment plans, and medical records using appropriate ICD coding (ICD-9/10).
- Engage in data collection and research activities related to patient outcomes to support continuous quality improvement initiatives.
- Educate clients on addiction medicine, relapse prevention strategies, and available community resources to promote sustained recovery.
- Provide wrap-around services to clients.
Qualifications
- Bachelor's degree in social work, psychology, counseling, or a related field; a master's degree is preferred.
- Valid licensure or certification in addiction counseling or clinical counseling (e.g., CADC, LCSW) is highly desirable.
- Proven experience in jail settings with expertise in substance use disorder treatment and mental health counseling.
- Strong knowledge of crisis management protocols, trauma-informed care (including PTSD), and behavioral therapy techniques such as applied behavior analysis.
- Familiarity with medical terminology, ICD coding systems (ICD-9/10), CPT coding for billing purposes, and electronic medical records systems.
- Excellent communication skills with the ability to perform patient assessments and deliver empathetic patient-centered care.
- Ability to work effectively within multidisciplinary teams while managing multiple priorities in fast-paced environments.
