Definition of Class

  • This position is responsible for the development and coordination of service support plans for transitioning clients and community based clients who are identified through the Group Violence Reduction Strategy (GVRS) and Reentry Support Services Alliance. Duties require working one-on-one with individuals to identify needs and connecting those individuals with the appropriate services to assure barriers are eliminated that would prevent follow through of service. Responsibilities include coordinating linkages and referrals to community providers for employment opportunities, education/training, housing, transportation, treatment services, monitoring and other comprehensive supports and services.

Nature of Work

  • Provides direct services to participants and technical assistance in a specialized area under moderate supervision. Represents the agency and program to professionals in the area of specialization through regular contact for the coordination of services to participants. Acts as case manager for assigned caseload. Facilitates the use of community resources. Develops and periodically reassesses case plans, making modifications as needed. Enforces program procedures and completes monthly program reports, as required. Work is performed in accordance with established procedures. Sets priorities and organizes work which is done under the moderate supervision and regular technical assistance. Maintains files in accordance with Program Guidelines and requirements, and prepares reports as required by supervisor.

Duties & Responsibilities

  • Works directly with transitioning clients and local service providers to develop a plan for reintegration from incarceration into the Erie community.
  • Works directly with GVRS targeted individuals to identify needs and develop service plans.
  • Works with program and parole departments to assist in supporting supervision requirements.
  • Represents the program and Agency at related meetings and conferences.
  • Ensures individual participant case planning encompasses key partners, support and services.
  • Maintains relationship and contracts within network of local service providers.
  • Maintains regular contact with participants to ensure continual progress through required follow-up period; documents contacts and case management services.
  • Works in concert with other program staff to achieve program goals and performance.
  • Responsible for oversight of participant records to ensure these records contain documentation required by funding stream requirements and regulations, and are current and in an orderly manner.
  • Safeguards, secures and otherwise protects GECAC property and equipment.
  • Adheres to confidentiality and security policies as required when working with client and partner information; ensures that all staff do the same.
  • Performs other duties as assigned by supervisor or by others in line of authority.

Knowledge, Skills, & Abilities

  • Must display a proven ability to plan and organize, and be able to work independently with minimal supervision. Must have excellent interpersonal communication skills, negotiation, mediation and conflict management skills. Requires good verbal and written communication and assessment skills. Must have ability to work with diverse groups, internal and external to the Agency. Must have basic working knowledge of computer systems, including word-processing, database, and spreadsheet software.

Minimum Requirements

  • Requires Bachelor’s Degree in Criminal Justice, Public Administration, Organizational Leadership or related field and two years of experience, or six years of experience in restorative justice, victim advocacy, offender services, or related field.
  • Must have experience supporting, empowering and managing individuals.
  • Must be willing to work with the prison population.
  • Must have valid PA driver’s license and full-time access to reliable, private transportation.
  • Must maintain appropriate auto insurance coverage as per agency policy.

Supervision Received

  • Work is performed under moderate supervision and regular technical assistance.

Conditions Of Employment

  • Must have the ability to adhere to confidentiality and security policies as required.
  • Must be able to work flexible schedule, which includes evenings, early mornings and weekends, as needed.
  • Must be able to work with persons of diverse backgrounds and the prison population.
  • Position must comply with PA Child Protective Services requirements, Act 33, 34 and FBI clearance required prior to hire. Updated clearances must be submitted every 36 months to maintain employment.
