Reentry Referral Specialist

Summary Objective

The purpose of this position is to work with identified population on determining appropriate interventions for success after reentry. Assist the THRIVE Fellowship Project Coordinator to determine appropriate applicants. Establish and maintain relationships with partnering agencies and organizations and serves as the liaison to the Office for Safe and Healthy Neighborhoods
This class works under general supervision, independently developing work methods and sequences.

Examples of Duties

ESSENTIAL FUNCTIONS

The essential functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the class as necessary.

  • Meets with fellowship team on a daily/weekly basis to share contacts, outcomes and ensure that all clients are being appropriately supported.

  • Assists in planning, developing and implementing procedures and programs.

  • Organizes and coordinates community programs, projects and activities with agencies or departments to provide information, to publicize available services and to maintain positive public relations as it relates to the fellowship.

  • Assesses and refers applicants and fellows to available financial, medical, social and community assistance and services.

  • Supports the review and selection of applicants for the THRIVE Fellowship program.

  • Works with Department of Corrections, Department of Probation and Parole, and Office of Safe and Healthy Neighborhoods in support of THRIVE Fellowship.

  • Develops internal and external relationships for the purposes of referral with service delivery providers, particularly education, behavioral health and employment and training agencies and resources.

  • Follows-up on identified population at Metro Corrections at specified intervals to ascertain release status, identify challenges, and connect them to available resources.

  • Performs related work as assigned.

Minimum Requirements

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

  • Bachelor’s Degree in Counseling, Criminal Justice, Social Science, Social Work or related discipline.

  • Two (2) years demonstrated effective work in community or an equivalent combination of education, training and experience.

OTHER MINIMUM REQUIREMENTS

  • Applicants and employees in positions which perform job duties that may require contact with offenders in the custody or supervision of the Department of Corrections or with youth in the care, custody, or supervision of the Youth Detention Services must meet qualifications pursuant to the federal Prison Rape Elimination Act, 28 CFR 115.17 and 115.317, to include periodic post- employment criminal background checks.

  • Must submit to a pre-employment polygraph test.

  • Must pass an annual tuberculin test.

  • Must provide one's own source of transportation.

  • Must not have been convicted of a felony.

  • Misdemeanor convictions will be considered on an individual basis.

  • Must possess and maintain a valid state driver’s license with an acceptable driving history.

MORE CENTER

  • Must submit to, and pass, annual criminal history checks.

PHYSICAL DEMANDS

Physical demands refer to the requirements for physical exertion and coordination of limb and body movement.

  • Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.

UNAVOIDABLE HAZARDS (WORK ENVIRONMENT)

Unavoidable hazards refer to the job conditions that may lead to injury or health hazards even though precautions have been taken.
