About the Role:

The Referral Coordinator plays a critical role in managing and streamlining the referral process within healthcare or service organizations to ensure timely and accurate patient or client transitions. This position is responsible for coordinating communication between patients, providers, and external agencies to facilitate seamless referrals and follow-ups. The Referral Coordinator ensures that all referral documentation is complete, compliant with regulatory standards, and entered accurately into tracking systems. By maintaining strong relationships with internal teams and external partners, the coordinator helps optimize service delivery and enhances overall client satisfaction. Ultimately, this role supports organizational goals by improving referral efficiency, reducing delays, and contributing to positive outcomes for all stakeholders involved.

Minimum Qualifications:

  • High school diploma or equivalent; associate degree or higher preferred.
  • Experience in a healthcare or social services environment, preferably in referral coordination or patient services.
  • Proficiency with electronic health records (EHR) systems and referral management software.
  • Strong organizational skills with attention to detail and the ability to manage multiple tasks simultaneously.
  • Excellent verbal and written communication skills.

Preferred Qualifications:

  • Certification in medical administration, healthcare coordination, or related fields.
  • Familiarity with insurance verification processes and healthcare billing procedures.
  • Experience working with diverse patient populations and understanding of healthcare regulations such as HIPAA.
  • Ability to analyze data and generate reports to support operational improvements.
  • Bilingual skills or proficiency in a second language relevant to the community served.

Responsibilities:

  • Manage incoming and outgoing referrals by verifying patient or client information and eligibility.
  • Coordinate with healthcare providers, insurance companies, and external agencies to facilitate timely referrals and approvals.
  • Maintain accurate and up-to-date records of all referral activities in electronic health record (EHR) or referral management systems.
  • Communicate effectively with patients or clients to provide updates, answer questions, and assist with scheduling appointments.
  • Ensure compliance with organizational policies, privacy regulations, and insurance requirements related to referrals.
  • Collaborate with internal departments such as case management, billing, and clinical teams to resolve referral-related issues.
  • Generate reports and track referral metrics to identify trends and opportunities for process improvement.
