Regional Clinical Coordinator - Wound Care Specialty Provider

Reports To: Chief Nursing Officer

Part-Time Only: Monday through Wednesday

This position is available due to continued growth – in this float position, you'll have the chance to make an impact across multiple communities, covering our partner facilities within several nearby counties to ensure continuity of exceptional care for our patients.

As a key member of our wound care team, you will:
➢ Conduct thorough assessments and develop personalized treatment plans that address underlying health issues and co-morbidities
➢ Deliver hands-on, bedside wound care to patients in skilled nursing and assisted living facilities
➢ Lead the charge on wound assessment, treatment planning, and advanced procedures
➢ Harness cutting-edge tech and evidence-based protocols to accelerate healing
➢ Guide smart, effective decisions based on a holistic approach in collaboration with facility teams and
families
➢ Keep your charting sharp and timely in our customized EMR
➢ Help drive quality initiatives and support our mission of raising the bar in post-acute care

Qualifications:

  • Active Pennsylvania Nurse Practitioner license (board-eligible not accepted) or Physician Assistant
  • Prescriptive Authority and DEA license – must obtain by end of training period
  • At least 2 years of wound care experience
  • Previous experience in SNFs, long-term care, or rehabilitation facilities preferred
  • Willingness to travel to designated partner facilities in several nearby counties
  • Comfortable with EMR, mobile, and laptop documentation
  • Strong interpersonal skills — collaborative, communicative, and adaptable

Pay: $93,000.00 - $96,000.00 per year
