Registered Dietitian (RD)

The Paramount at Somers Rehabilitation and Nursing Center is looking for an ambitious and dedicated Registered Dietitian (RD) to join our ever-growing team.

Registered Dietitian (RD) Job Summary:

• Compile and utilize dietary and nutrition care data and any other pertinent data to complete annual and quarterly nutritional assessments, develop a plan of care based on assessments of their nutritional needs, and formulating appropriate individualized goals

• Evaluate and interpret, review and modify plans of care, and deliver nutrition care to residents

• Provide routine visits to monitor effectiveness of residents’ diets, meal consumption, and nutritional interventions for appropriateness

• Attend quarterly and annual evaluations and care conference meetings

• Participate in wound rounds, UR, QAPI, discharge planning, and other various interdisciplinary meetings and rounds

• Provide nutrition education to patients and families

• Conduct meal rounds, test trays, menu checks, and other services as needed

• Precept Dietetic Interns and instruct them on the competencies required to complete the assigned rotation, providing feedback for incorporation into student evaluations

• Assist co-workers as needed and maintain positive interpersonal relationships with colleagues

Skills

• Able to work alongside interdisciplinary team, exhibit strong communication skills as well as work independently on daily tasks

• Good understanding of how document in EMR (Point Click Care is a plus) and menu system (knowledge of MealTracker is a plus)

• Ability to teach others, specifically pertaining to nutrition and dietetics.

Registered Dietitian (RD) Requirements

• Must be a Registered Dietitian

• Ideal if with at least 1 year of healthcare experience, nursing home experience a plus

• Full-time

• This is an in person on site job (no remote work)
