Program: HELP One, Bldg. C |314 Hinsdale Street, Bklyn, NY 11207
Position Summary
As a Registered Nurse for our newly constructed supportive housing program in Brooklyn (HELP I), you’ll report to the Executive Director, you’ll coordinate health care services for 111 families with children headed by young adults aged 18 to 25 years, and 43 single adults with mental health and/or substance use issues from the NY 15 15 program. Your responsibilities will include:
- Assisting residents in managing medications and navigating appointments and referrals.
- Providing residents with health education on chronic disease management and preventative health strategies.
- Providing staff training on health and mental health issues.
- Assisting with acute care coordination.
Requirements:- Licensed Registered Nurse.
- 5 years of experience working with special needs populations.
Compensation
Actual compensation will not be less than the applicable minimum wage, prevailing wage, or exempt salary requirements under federal, state and local laws.
