The Registered Nurse (RN) plays a vital role in ensuring the health, safety, and well-being of individuals with intellectual and developmental disabilities. Using the full nursing process—assessment, planning, intervention, implementation, and evaluation—the RN delivers compassionate, person-centered care across assigned homes and programs. This role collaborates closely with families, healthcare providers, and interdisciplinary team members to support each client’s unique needs, promote independence, and uphold the highest standards of professional nursing practice. As an essential part of the HGH care team, the RN helps create an environment where every individual can thrive with dignity, respect, and quality of life.
Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of clients’ care, as evidenced by client/staff observation.
Assesses clients’ health care needs; collects and documents current health status.
Makes accurate and ongoing assessments of clients’ status and responds appropriately.
Evaluates and analyzes clinical situations and makes decisions within the scope of licensed practice.
Performs evaluation of new and existing clients to determine treatment priorities and need for modifications.
Demonstrates necessary skills and knowledge to provide care tailored to each client’s needs.
Demonstrates ability to handle emergency situations promptly, precisely, and professionally.
Sets priorities for nursing action in a logical sequence based on clients’ needs and medical providers’ orders.
Formulates individualized plans of care.
Performs ongoing assessment and revises plans of care based on new data or changes in clients’ conditions.
Ensures all clinical activities are carried out in accordance with each client's Health Care Plan.
Provides personal client care to promote comfort, dignity, and well-being while acknowledging physiological and psychological needs.
Observes and documents clients’ responses to medications.
Documents accurate, legible, and timely assessments of client status.
Records nursing interventions, client responses, needs, problems, capabilities, limitations, and progress toward goals.
Includes documentation of all relevant client/staff teaching and their demonstrated understanding.
Maintains up-to-date information on appointments, procedures, and health-related updates in each client’s chart.
Compiles data for periodic assessment of physical condition and evaluation of health care effectiveness.
Documents all communication with physicians, families, case managers, and program staff.
Notifies and documents any changes in client condition to all appropriate parties.
Ensures accurate transcription and follow-through of physician orders per policy.
Trains clients, staff, and others on appropriate health information in a timely manner; shares educational materials as needed.
Promotes effective communication among all staff members and communicates pertinent health information to appropriate personnel.
Performs direct care utilizing established procedures, policies, and standards as evidenced by client/staff feedback.
Valid Registered Nurse (RN) license in the State of California.
Current CPR/First Aid certification for Healthcare Professionals.
Experience providing services to individuals with intellectual and developmental disabilities preferred.
Knowledge of HIPAA guidelines and patient privacy regulations (required).
Ability to read, write, and understand the English language.
Ability to multi-task, prioritize, and manage time efficiently.
Able to establish and maintain effective, professional relationships with clients, coworkers, neighbors, volunteers, vendors, and the public.
Demonstrates patience and the ability to tolerate and navigate challenging or frustrating situations with professionalism and calm.
Ability to drive an agency vehicle safely, lawfully, and confidently in various traffic and weather conditions.

Physical Requirements/Work Environment
This job operates in an office and in residential environments. While performing the duties of the job, the employee is regularly required to see, talk, hear, walk, bend, stoop, sit, stand, and reach. The employee is frequently required to stand, walk, and sit. The employee may frequently move to interact with fellow employees and/or clients. Additionally, the individual may be required to lift objects weighing up to 30 pounds.

We support children and adults in San Diego and Imperial Counties with intellectual and developmental disabilities, such as Down syndrome, epilepsy, autism, and cerebral palsy. We provide a full continuum of care including residential care in specialized group homes, independent living support, respite care, and adult foster home services. Additionally, we provide early childhood assessments and services, tailored day support, Life Planning workshops, county-wide transportation services, and on-site activities at our resource center.
