Registered Nurse

S & S Home Health Care Agency is seeking a compassionate, detail-oriented, and dedicated Part-Time Registered Nurse to join our dynamic team. In this role, you will provide high-quality, patient-centered nursing care in a home health setting while working closely with the Director of Nursing (DON). Your primary responsibilities will include conducting skilled nursing visits, assisting with patient admissions, and supporting clinical documentation to ensure compliance with agency protocols and regulatory standards.

As a key clinical team member, you will collaborate with interdisciplinary staff to develop and implement individualized care plans that promote the best outcomes for our patients. Key Responsibilities:

• Perform in-home skilled nursing visits, assessments, and evaluations for patients.

• Assist the Director of Nursing with patient admissions, including initial assessments, care planning, and coordination.

• Document patient care accurately and timely in accordance with agency policies and state/federal regulations.

• Monitor patient conditions, communicate changes to the DON and physician, and recommend interventions as needed.

• Educate patients and caregivers on disease management, medications, and home care procedures.

• Ensure patient safety and adherence to infection control standards during home visits.

• Collaborate with other healthcare professionals to ensure continuity and quality of care.

Qualifications:

• Current Registered Nurse (RN) license in the state of practice.

• Minimum of one year of nursing experience, preferably in home health or a community-based setting.

• Strong clinical assessment, documentation, and organizational skills.

• Ability to work independently and manage a flexible, part-time schedule.

• Valid driver’s license and reliable transportation.
