Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 24 months of Home Health experience.

Experience with McKesson is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Vermont license or compact license

Certifications

BLS (Required)

Vaccinations

Flu vaccination required. Religious, medical, and personal declinations accepted.

Proof of Identification

Driver's license required

Required Skills/Experience:

Serves as Case Manager for assigned patients and actively manages case loads and visit times in accordance with agency productivity standards.
Stays up-to-date on a variety of High-Tech skills; IV use and maintenance, wound VAC, complex wound care

Shift & Scheduling: Monday-Friday. 8am-4pm with some weekend and on-call required. Holiday Coverage Required: Yes

Floating Requirements: Hospice coverage required when on-call.
