Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 24 months of Case Management experience.
3 months of experience with Epic is preferred from any number of months.
References
Candidates must provide 2 references from any number of months in order to be considered for this role
License
Candidates must have a New Jersey license or compact license
Certifications
BLS (Required)

Additional Details:
Required skills and experience: Prior clinical experience in care and management of hospitalized patients.
Preferred skills and experience: Experience in acute care case management. Case management training from a professional Case Management organization, preferred.
