JOB DESCRIPTION (MAIN JOB TASKS)

OVERALL RESPONSIBILITIES

· Adhere and comply with Organization Regulations and Administrative Policies.

· Adhere and comply with policies and procedure related to patient care, infection control, patient safety and accreditation standard.

· Implements patient care, plan as per doctor's recommendation; monitor & record vital parameters.

· Monitors, record & report any symptoms / changes in the patient condition to the doctors.

· Ensures patient safety by adhering to International Patient Safety Goals (IPSG)

· Accompanies the doctor on rounds: - provide update on patient condition and help in clinical procedures.

· Maintains continuity of patient care through documentation & communication during shift change.

· Remains responsible for consumables: - for the patients under her nursing care, accounting and indenting of consumables/ medicines & nursing supplies and ensuring the availability of the same.

· Educates patients / patient relatives on medication and self-care during stay and discharge as necessary for recovery.

· Ensure regular relevant and appropriate health education is given to all patients/relatives assigned to her care on a daily basis.

· Updates on essential knowledge and skill by participating in on-going CNE (continuing nursing education), on the job trainings and various other academic training programs organized

· Ensures financial clearance in full before discharge of patient from the ward.

· Ensures all orders, admission/discharge/death/bills/diet/transfer etc. are up-to-date in HIS.

· Reports Expiry dates of Drugs and working condition of equipments to appropriate person/incharge to Keep everything in working order.

· Reports loss or damage of equipments during duty hours to the reporting manager.

· Knowledgeable about the medication process and ADR.

· Administers prescribed medications safely and accurately by following rights and follows the hospital policy on safe drug administration.

· Reports any medication error, ADRs and accurately complete incident report or ADR forms.

· Adheres to the Incident management policy of the organization. Raises incident reports when required. Has understanding on Root Cause Analysis (RCA) and Corrective & Preventive Action (CAPA) of incidents

· Knowledgeable on the nursing quality indicators

· Performs the 5 moments of hand hygiene.

· Ensures appropriate cleaning and fogging of the patient room after care of infected cases.

· Notifies the infection control department about the notifiable diseases.

· Proper segregation and disposable of BMW after medication administration or any other procedure as per hospital infection control policy

· Supervise the nursing aides when they are attending the personal hygiene of the patient’s.

· On admission receive the patient and make him / her comfortable. See that the beds are made prior to the patient’s entry to wards.

· Ensures Room Orientation is given to the patients and relatives on admission.

· Ensures Dietary information is given to the patient/relative. Informs the café staff as and when required

· Ensures Special attention to vulnerable patients, patients on restrains and patients at risk categories.

· Responsible for Initial assessment to be completed within 30 mts of admission & reassessment in each shift.

· Warrants that all patient care plans are done and documented according to the patients need and condition & the same is modified as per the condition of patients, to ensure proper continuity of care

· Checks and documents patient’s Vital signs & pain assessment every 4 hourly & as when required.

· Ensures all patient information is appropriately documented and handing over is done at the end of each shift & during transfer between unit / departments following ISBAR

· Inserts peripheral venous catheters as and when required as per safe infusion practice protocol

· Mention date and time of insertion and arrow indicating the direction of the vein flow.

· Check and send correct specimen to the lab following the hospital policy.

· Collect the lab reports in time and inform the concerned consultant.

· Check for chemotherapy patients —chemo-plan and ensures the same is written completely by the respective consultant, before the start of chemotherapy.

· Checks for consent and check list which needs to be filled before each chemo-cycle is started.

· Checks pre-op patient for local site preparation, site marking, investigation reports, anesthesiologists’ pre-op check and premedication orders and complete it.

· Carry out the protocol for Blood & blood products transfusion

· On patient’s discharge, see that the discharge procedure is carried out as per the policy.

· Enter details in the activity card immediately after each procedure.

· Knowledgeable about all codes & how to raise emergency codes.

· Well versed in EMR and complies to it.

TRAINING AND DEVELOPMENT

· Utilizes effective communication skills in reporting pertinent information on time.

· Adheres to the grooming standards.

· Aware of the departmental vision, mission, objectives and its implementation.

· Participates in the hospital quality improvement program.

· Works collaboratively and support efforts of team members.

· Completes BLS/ACLS/PALS/NALS certification (as applicable).

· Demonstrates exceptional customer service and interact effectively with physicians, patients, visitors, staff and the broader health care community.

· Participates in the academic programs and CNE which is planned for nurses in order to upgrade both knowledge & skills.

QUALITY

· Updates and keep abreast of the HAI statistics and preventive measures.

· Handles the sharp items safely and disposes the sharps properly.

· Works as a team to enhance patient satisfaction and physician satisfaction

· Knowledgeable on nursing quality indicators and incident management policy
