Registered Nurse Advocate and Care Manager

We are seeking an experienced Registered Nurse to join our dynamic healthcare team. In this vital role, you will serve as a patient advocate, guiding individuals through complex healthcare systems while providing additional support as needed. Your primary focus will be on coordinating comprehensive care plans, facilitating effective communication among multidisciplinary teams, and ensuring patients receive personalized, high-quality services across inpatient and outpatient settings. This role offers an exciting opportunity to make a meaningful difference in patients’ lives by promoting health, safety, and well-being through proactive management and advocacy.

This key role will be responsible for all aspects of Care Coordination and Advocacy for their own client caseload, consisting of patients with a wide variety of acute and chronic healthcare needs across the lifespan. The role may combine visiting patients in their residence or remotely to manage the many aspects of care coordination and if needed, to provide hands on skilled nursing care.

Major Responsibilities: Some of the primary responsibilities include the following:

  • New consults and admission of new patients
  • Management comprehensive plans of care
  • Ongoing patient physical and psychological assessments
  • Medication management, in-home assessments
  • Professional communication between the comprehensive medical/care team, patient and loved ones for continuity of care
  • Ongoing patient, family, and caregiver education
  • Referrals and recommendations based on patient needs, especially connecting with community resources
  • Regular review and maintenance of electronic medical records
  • Participation in ongoing primary care conferences
  • Documentation
  • Other responsibilities as neede

Job Requirements: Applicants for this position must have the following skills and experience:

  • Current Registered Nurse licensed to practice in the state of California
  • Graduate of an approved and accredited nursing program
  • Preferred 10 years nursing experience, including acute care, but will consider a strong candidate with less experience
  • Strong patient assessment skills
  • Strong organizational skills and ability to prioritize tasks quickly and effectively
  • Current BLS certification
  • Outstanding written and verbal communication skills
  • Ability to be a strong yet professional and tactful representative for our patients
  • Ability to work independently and efficiently
  • Ability to use iPhone as a professional technology tool
  • Ability to use applications on phone or computer
  • Ability to leverage AI as appropriate
  • Computer literate with strong computer skills and comfortable with AI
  • Valid CA driver’s license, current automobile insurance and access to a reliable vehicle
  • RN liability insurance
  • RN Case Management/Home Health/Hospice/Community RN experience a plus
  • Certification in Case Management (i.e., through CMSA) a plus

Pay: $45.00 - $70.00 per hour
