Position Summary:

Registered Nurse (RN) CAP Assessor works with Case Managers and recipients of In-Home Aide services to ensure that all identified needs are met to the best of our ability in regard to the specific program that the clients fall under.

Essential Job Functions:

  • Handle all CNR’s (annual re-certifications) per program guidelines.
  • Follow up on status change assessments.
  • Update electronic records (e.g. Careworks, Ecap etc).
  • Respond to inquiries, complaints, and incidents as necessary.

· Conducts biopsychosocial assessments to determine appropriate CAP/ DA Level of care monthly.

· Responds to inquiries, complaints, and incidents as necessary.

· Locates community resources on a need basis for recipients.

· Participates in multi-disciplinary team meetings monthly, If needed ( add)

· Maintains program deadlines and accurate up to date documentation in the ECAP service record in accordance with state and agency policies.

· Ensures compliance by fulfilling all licensure, accrediting, program requirements.

· All other duties as properly assigned.

Knowledge/Skills/Abilities:

  • Good written and verbal communication skills
  • Basic computer skills
  • Current Driver’s License and clean driving record
  • Strong interpersonal skills
  • Time management/organizational skills

Education/Experience/Training:

  • Current RN NC License in good standing
  • At least 3 years’ experience preferably working with seniors

Schedule:

  • Remote; 20.0 hours per week, Monday through Friday
