Duties
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine appropriate care strategies.
- Coordinate inpatient and outpatient care plans by collaborating with multidisciplinary teams across hospital, post-acute, and clinic settings.
- Manage discharge planning processes to ensure smooth transitions from hospital to home or other care facilities, including arranging necessary services and follow-up appointments.
- Utilize utilization management principles to evaluate medical necessity and optimize resource use while maintaining compliance with HIPAA regulations.
- Perform diagnostic evaluations and case management activities for diverse patient populations such as geriatrics, pediatrics, behavioral health patients, and those with infectious diseases or trauma-related conditions.
- Oversee patient records documentation accurately within electronic medical records systems, ensuring adherence to medical terminology standards and privacy laws.
- Engage in patient service activities by providing education on health conditions, medication management, and post-care instructions to promote self-care and recovery.
Skills
- Extensive critical care experience including ICU, emergency room, Level I Trauma Center, Level II Trauma Center, or Level III Trauma Center settings.
- Proven expertise in hospital medicine, medical-surgical units, post-acute care environments, outpatient clinics, or managed care programs.
- Strong knowledge of physiology, diagnostic evaluation techniques, and clinical assessment tools essential for effective patient evaluation.
- Experience with discharge planning, case management strategies, utilization management processes, and medical records documentation.
- Familiarity with pediatric (PICU), geriatrics (Geriatrics), behavioral health (Behavioral health), infectious disease care (Infectious disease), and OR experience is highly desirable.
- Ability to perform patient assessments accurately while demonstrating excellent patient service skills in fast-paced inpatient or outpatient settings.
- Proficiency in medical terminology and understanding of HIPAA compliance to safeguard patient information at all times.
