Job Summary
We are seeking a dedicated Registered Nurse Case Manager to coordinate patient care within a dynamic healthcare environment. The ideal candidate will facilitate seamless discharge planning, manage utilization, and ensure optimal patient outcomes across various inpatient and outpatient settings. This role requires a strong understanding of hospital operations, medical terminology, and patient-centered care, particularly within trauma centers, ICU, and behavioral health units. The Hospital Case Manager will serve as a vital link between patients, families, healthcare providers, and community resources to promote effective recovery and post-acute care transitions.
Duties
- Conduct comprehensive patient assessments to determine medical needs and discharge requirements.
- Develop individualized care plans that include discharge planning, post-acute care arrangements, and resource coordination.
- Collaborate with multidisciplinary teams across hospital departments such as ICU, PICU, trauma centers (Level I & II), emergency medicine, and behavioral health units.
- Manage utilization review processes to ensure appropriate level of care while complying with HIPAA regulations.
- Coordinate with social workers, addiction counselors, and mental health professionals to support patients' psychosocial needs.
- Facilitate communication between patients, families, insurance providers, and community agencies for smooth transitions of care.
- Maintain accurate medical records and documentation related to patient care plans and case management activities.
- Assist with intake procedures and diagnostic evaluations for inpatient and outpatient populations.
- Support discharge planning efforts by arranging outpatient services, home health care, or other post-discharge resources.
- Stay informed on hospital policies, managed care protocols, and relevant healthcare regulations to ensure compliance.
Skills
- Graduate of a nursing program. BSN preferred.
- Licensed Registered Nurse in the state of Pennsylvania.
- Proven experience in hospital case management within acute care settings such as ICU, trauma centers (Level I & II), or post-acute environments.
- Strong knowledge of medical terminology, physiology, diagnostic evaluation, and patient assessment techniques.
- Familiarity with HIPAA regulations and medical records management.
- Experience in discharge planning, utilization management, and managed care processes.
- Background in behavioral health or mental health counseling is advantageous.
- Excellent communication skills for effective collaboration with multidisciplinary teams and patients’ families.
- Ability to handle complex cases involving addiction counseling or social work interventions.
- Proficiency in patient service delivery across inpatient and outpatient clinics; experience with pediatrics or post-acute care is a plus.
- Strong organizational skills with attention to detail in documentation and case tracking.
