JOB DESCRIPTION

FIELD REGISTERED NURSE CASE MANAGER

I. QUALIFICATIONS:

A. Graduate of an approved school of professional nursing.

B. Two (2) years of general medical-surgical nursing preferred.

C. Minimum of one (1) year of nursing experience in home health or community health preferred.

D. Recipient of a current license by the State Board of Nurse Examiners.

II. JOB KNOWLEDGE:

A. Ability to be self directed and function independently. Demonstrate problem solving and critical thinking skills.

B. Ability to understand the physical, psychological and spiritual concerns/problems of terminally ill patients and families.

C. Ability to assess, evaluate and perform the necessary clinical skills involved in providing skilled nursing care to patients in the home setting.

D. Demonstrate understanding of disease progression and dying processes and expected changes.

E. Ability to work consistently under pressure, and adapt to changes with scheduling.

To be objective, to keep feelings, prejudices and personal problems from having a negative impact on work performances.

F. Ability to work effectively within an interdisciplinary team in both long and short- term relationships, to maintain smooth relationships.

III. FUNCTIONS:

A. Conducts a thorough holistic assessment on admission of the patient, and on a weekly basis thereafter. Complete admission assessment and documentation as needed.

B. Coordinates services for the care of the hospice patients.

C. Assumes primary responsibility for the physical care of the patient in coordination with the attending physician's orders.

D. Provides frequent information to the attending physician regarding the condition of the patient.

E. Documents visits and phone calls on an ongoing basis for the clinical record. All documentation to be completed according to time frame set in policy.

F. Participates in the QAPI program and documents pertinent issues. Examples: falls, infections control issues, complaints

G. Makes necessary referrals to social worker/counselor when needed. Responsible for notifying social worker/counselor when significant changes are made.

H. Attend continuing education programs per licensure and provides proof to Human

Resources.

I. Availability-meets attendance standards; is punctual; works when needed, responsible for checking schedule and covering missed shifts. All staff are responsible for covering their shifts when scheduling vacation or requesting time off. Supervising nurse will assist with shift coverage in the event of an emergency or inability to cover shift. ______initials

J. Rotates on-call coverage with other staff nurses as necessary. ______ initials

K. Makes supervisory visits for patient care aides every 7 days per Hospice in the

Pines policy and LVN Supervisory note each LVN visit. ______ initials

L. Prepares the Patient Care Aide Careplan individualized to patient, and update

PCA & RN case manager with any changes in patient’s status.

M. Assists with coordinating community resources and planning for alternate care when patient can no longer remain at home.

N. Reviews, updates and evaluates the IDT Plan of Care, MAR, PCA Care plan as frequently as the severity of the patient's condition requires and/or per policy.

O. Participates in team support groups and activities.

III. FUNCTIONS: (Continued)

P. Provides support to family members in the period of bereavement in coordination with other team members.

Q. Assumes responsibility for the development and implementation of

Interdisciplinary Care Plan on assigned patients.

R. Reports changes with patient information via Change of Status in a timely manner to Medical Records Coordinator to ensure appropriate billing.

S. Reports to on-call nurse all pertinent changes and circumstances in patients.

Reports to be given by 4:30 daily if at all possible.

T. Monitors and reviews documentation on assigned patient's electronic charts to assure that care is complete, accurate and timely.

U. Once PCA assignment is written, coordinates aide visits with patient/family.

Responsible for notifying PCA of significant changes in patient’s condition.

V. Functions as Office Nurse as assigned.

W. Completes activity sheets on a daily basis. Monitors overtime to limit overtime and stay within hospice budget.

X. Maintains adequate inventory of medical supplies and equipment.

Y. Informs patients and families of opportunities to help Hospice program by designating Hospice as recipient of memorial donations, donating goods to be distributed to needy families, and volunteering their time and talent.

Z. All other duties as assigned.

IV. ENVIRONMENTAL AND PHYSICAL REQUIREMENTS FOR CLINICAL

POSITIONS (to perform essential functions):

A. Must be able to work in diverse settings, to include possible contact with blood borne pathogens, respiratory hazards, electrical hazards, and appropriate precautionary actions.

B. Must be able to work in potentially volatile situations, such as being verbally confronted by hostile individuals, patients, families, to include derogatory language.

C. Must be able to work alone doing repetitive tasks for extended periods of time to include standing, bending, etc.

D. Must be able to work in high traffic area without disruption.

E. Must be able to stand, walk, lift, pull, and push.

F. Must be able to speak English, have good near vision, hear and smell for sensory purpose.

G. Must be able to distinguish colors to verify proper medical condition for assessment purpose.

H. Must be able to climb stairs and ladders, for job requirements.

I. Must be able to perform repetitive motions, stooping, bending, grasping, reaching, kneeling, carrying, and typing/ word processing.

Pay: $35.00 - $38.00 per hour
