ESSENTIAL JOB FUNCTIONS

Clinical Services
Provide nursing services within scope of practice, including:
Provide direct clinical care to deliver patient-centered clinical services, including assessment, diagnosis, treatment, and follow-up, within the scope of family medicine and public health nursing.
Follow standing orders, clinical protocols, Public Health policies and protocols in conjuncture to applicable Mesa County policies. Adhere to privacy/confidentiality standards in all client interactions.
Safely administer medications and treatments as ordered, according to policy.
Maintain knowledge of immunization schedules, STI treatment protocols, and public health policies.
Provide patient education on preventive health, medication use, disease prevention and management, coordinate follow-up care, lab results and referrals internally to other Public Health programs or externally to community providers as deemed appropriate for disease process.
Administer routine and travel immunizations to children and adults per CDC/ACIP guidelines.
Assist providers in specific care models for reproductive health services in accordance with Title X program guidelines, family medicine and clinic policy and protocols.
Provide reproductive health services, including birth control counseling, STI testing and treatment, pregnancy testing, and interdisciplinary and community referrals as deemed appropriate patients care needs. Support family planning services, including patient education and contraceptive counseling under established protocols and statues..
Maintain accurate and timely documentation in electronic health records
Triage patients in person, phone or telemedicine, using clinical judgment to support appropriate care decisions.
Implement evidence-based practice with patient in coordination with the healthcare team. Develop, modify patient care and treatment plans as indicated by patient’s responses and conditions. Provide safe, appropriate nursing care and patient education in an ambulatory community based clinic setting.
Perform needed nursing assessments – Collects comprehensive data pertinent to the patient situation. Assess the needs of individuals, families and/or communities to identify potential health or safety problems by Collaborating with nurse practitioners, midwives, and other providers to support comprehensive care delivery.
Communicates findings and treatment education to patients with appropriate and efficient documentation. Patient interaction may be collected as the result of a face to face, video, or telephone encounter.
Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care. Advocates for patients and family in accordance with ANA code ethics and professional practice standards. Prepare patients for, and assist with examinations and treatments Nursing Diagnosis analyze assessment data to determine nursing diagnosis.
Community-Based Outreach
Plan and participate in outreach activities to engage diverse and underserved populations, including attending community events, schools, and partner organizations. (e.g. mobile clinics, school-based events, and pop-up immunization sites).
Promote health education and awareness of available services to improve access and equity in care delivery.
Participate in outreach events, mobile clinics, and health fairs to promote immunizations and reproductive wellness.
Provide culturally sensitive education to diverse populations regarding vaccine-preventable diseases, sexual health, and family planning.
Collaborate with schools, community organizations, and healthcare providers to increase access to services and improve health outcomes.
Health Promotion and Disease Prevention
Educate individuals and families on preventive health practices, chronic disease management, and wellness strategies.
Implementation and interdisciplinary collaboration of Public Health Programs
Make appropriate referrals to WIC, Nurse-Family Partnership and other relevant services.
Tracks patient data according to required deliverables for different grants.
Professional Development
Participate in quality improvement activities, staff meetings, and required trainings.
Maintain state licensure as required.
NON-ESSENTIAL JOB FUNCTIONS
Participates in emergency planning and response activities.
Participates in agency-wide activities that may include strategic planning, quality improvement, and/or staff development, as appropriate.
Completes necessary training through FEMA, including Incident Command Structure (ICS) courses.
When a local declaration of emergency or disaster is declared by County leadership, employees may be required to work as part of a National Incident Management System (NIMS).
Performs other duties as assigned.
MINIMUM QUALIFICATIONS REQUIRED
Education and Experience
Associate’s degree in nursing (ADN) or Bachelor’s degree in nursing (BSN), with two years experience in community or public health care experience.
Familiarity with Title X guidelines, immunization protocols, and STI treatment algorithms preferred.
Bilingual (Spanish/English) Strongly preferred.
Experience working with grant-supported or underserved populations.
Certificates and Licenses
Current license to practice as a Registered Nurse in Colorado.
A valid Colorado driver’s license with a responsible driving record.
Current Basic Life Support (BLS) certification
Computer and Technology
Ability to use Google applications and Microsoft Office applications as needed to perform various job tasks.
Ability to use different EHR systems such as but not limited to Colorado Immunization Information System, CIIS, Quality Health Network (QHN), Allscripts, Eclinicals, etc.
PHYSICAL REQUIREMENTS and WORKING ENVIRONMENT
Primarily clinic-based with occasional off-site community events or mobile outreach activities.
Some evening or weekend hours may be required based on community needs or grant deliverables at different times throughout the year.
This position requires standing, stooping, sitting, bending, twisting, and lifting up to 10 pounds.
Work is generally confined to a standard office environment.
May be exposed to communicable diseases and infections.
