JOB SUMMARY
The AFC provider must employ or independently contract with a registered nurse. The registered nurse
may function as the program director.
RESPONSIBILITIES
Completing and coordinating all clinical assessments and clinical evaluations.
Developing, implementing, and reviewing on an ongoing basis each member’s AFC plan of
care.
Developing the member’s interim and final AFC plan of care, with input from the member or
responsible party, all members of the MDT, and other individuals designated by the member
Overseeing, training, evaluating, and supervising AFC caregivers in conjunction with the care
manager.
Reviewing the PCP Summary Form.
Coordinating and implementing the PCP form and approval for AFC with the member, AFC
caregiver, and AFC provider personnel.
Monitoring the health status of all members to ensure that all needed AFC is properly delivered.
Reporting changes in the health status of any member to the member’s PCP.
Coordinating the delivery of AFC with any other health services or supportive services the
member is receiving from MassHealth or other agencies or organizations
Reviewing AFC caregiver logs at a minimum of every 30 days.
Conducting on-site visits with each member at the member’s qualified setting in accordance with
level of care.
Completing a nursing progress note corresponding with each on-site visit or encounter, or more
often as the member’s condition warrants.
Educating the member about hygiene and health concerns.
Completing and submitting semiannual health-status reports to the member’s PCP.
Developing, in conjunction with the MDT the AFC caregiver, and the member or responsible
party, an emergency backup and personal care contingency plan for each member receiving
AFC that includes an alternative plan for the member if the AFC caregiver is temporarily unable
to provide care.
Planning for and implementing discharges and transition from the AFC program.
Conducting an orientation for each AFC caregiver before the AFC caregiver begins personal
care.
Providing ongoing training to AFC caregivers on health and aging.
QUALIFICATIONS
The registered nurse must be fully licensed by the Massachusetts Board of Registration in Nursing.
The registered nurse must have at least two years of recent experience in the direct care of elders or adults with disabilities.

Requires using hands and fingers to type on a keyboard and navigate a computer mouse
Requires eyesight to see paper documents and computer monitor

Requires face-to-face verbal discussions with individuals or teams
