Duties
- Provide comprehensive home health care to adult and geriatric patients.
- Conduct admission assessments, routine visits, and start of care.
- Manage a caseload of 5-6 patients per day, ensuring timely completion of visits within an 8-hour shift.
- Perform wound care, including pressure ulcer prevention and management.
- Change PICC line dressings and manage peripheral IVs/central lines/ports.
- Administer medications and manage pain assessments using verbal and nonverbal scales.
- Utilize OASIS for documentation and assessment.
- Provide education on nebulizer use and oxygen safety.
- Manage urinary and suprapubic catheters.

Suprapubic Catheter management*
- Minimum of 1 year of experience required, 2 years preferred.
- Experience with high volume caseloads and acute patient care.
- Travel experience required.
- Recent OASIS experience required, with an exam to be taken prior to interview.
- MatrixCare EMR experience preferred.
- Service area includes LA County with a mileage radius of 5-20 miles.
- Weekend coverage required every 4 weeks.
