Registered Nurse (in home assessments)

Job Description

We are seeking a reliable and compassionate Registered Nurse (RN) to conduct in-home nursing assessments for clients receiving home care services in Richmond, Henrico, Chesterfield and nearby County. This role focuses on initial and follow-up assessments, documentation, and communication with our care coordination team. No routine bedside or shift work is required.

This position is ideal for an RN looking for flexible, per-visit work in a field-based setting.

Responsibilities

• Perform in-home nursing assessments for new and existing clients
• Complete all required assessment documentation accurately and on time
• Observe and report changes in client condition
• Coordinate findings with administrative and care teams
• Ensure compliance with Virginia state regulations and Medicaid requirements
• Maintain professional and respectful interaction with clients and families

Qualifications

• Active Registered Nurse (RN) license in the State of Virginia
• Minimum 1 year of nursing experience (home health or community nursing preferred)
• Strong assessment and documentation skills
• Ability to work independently and manage visit schedules efficiently
• Reliable transportation and valid driver’s license
• Comfortable with electronic documentation systems
