Key Responsibilities

• Provide skilled nursing care in patients' homes
• Assess, monitor, and document patient condition
• Administer treatments and follow individualized care plans
• Communicate and collaborate with care team and families, and the healthcare team

Qualifications

• Active Colorado RN license (1+ year preferred)
• Minimum of one (1) year of nursing experience preferred
• Current CPR/BLS certification
• Valid driver’s license & reliable transportation
• Strong clinical skills, professionalism, and compassion
• Ability to work independently while maintaining high-quality patient care
