Registered Nurse - Per Diem LHCSA

RESPONSIBILITIES:

  • Assesses new cases for need and level of home care/nursing care required. Plans for comprehensive service to patients and their families: develops nursing care plans, including goals in conjunction with the patient’s physician. Coordinates the patient’s plan of care, including Home Health Aide and Personal Care Aide assignments. Evaluates the effectiveness of care plan, modifying the care plan as indicated. Communicates with the physician regarding the patient’s condition and/or progress. Provides nursing care as follows:
  • Teaches and supervises the patient, family, PCA/HHA, and others providing nursing care or personal care to the patient.
  • Provides or assists with preventive and therapeutic treatments under a medical provider’s direction. This may include patient transfers.
  • Initiates nursing measures to prevent complications to minimize disabilities.
  • Serves as the Case Manager/primary nurse to a caseload of patients.
  • Performs PRI/Screen and/or UAS as indicated.
  • Supervises care provided by Home Health Aides, Companions and PCA’s and Private Duty Nurses as required. Assures that HHA services are consistent with agency vision, mission, values and strategic goals and meets customer’s needs. Delivers HHA/PCA evaluations in a timely manner.
  • Responsible for approval of HHA/PCA time off requests and submission to payroll
  • Responsible for follow up on payroll questions by paraprofessional staff
  • Responsible for the disciplinary and corrective action plans as it relates to the paraprofessional clinical job performance.
  • Assists paraprofessional staff with documentation questions and fixes
  • Promotes good health practices to include understanding of diagnosis and implications involved. Promotes independence in self care. Provides information about and the coordination of available community resources.
  • Develops and revises comprehensive and coordinated plan of care in conjunction with patient, family, medical provider, care delivery team and third-party payers including obtaining medical orders and approval of medical services.
  • Maintains current knowledge of all available URMHC/Signature Care, MLTC and community-based services.
  • Supervises care and the care plan provided by Personal Care Aides/Home Health Aides.
  • Maintains timely and accurate reports and records, adhering to the agency standards.
  • Ensures the timely and appropriate resolution of all questions, complaints, and emergencies per agency standards
  • Participates in activities that promote agency and staff growth and development,
  • Participates in Quality initiatives, including clinical record audits and accrediting body surveys.
  • Participates in programs for the development of clinical expertise.
  • Participates on committees as assigned.
  • Assures that care delivery services are consistent with the ICARE goals, agency vision and customer needs and investigates patient complaints and incidents as assigned and develops improvement plan as indicated.
  • Responsible for the Key Performance Indicators (KPI’s) established by leadership including timeliness of documentation and productivity standards.
  • Responsible for managing assigned Work Ques and Reports in eRecord related to paraprofessionals, as assigned
  • Participates in scheduling of paraprofessional staff in collaboration with PSC’s and LHCSA Manager
  • Serves as back up the LHCSA Manager
  • Other duties as assigned.

QUALIFICATIONS:

  • Current NYS RN licensure in good standing. PRI/Screen and UAS certification preferred, but will train.
  • Minimum of one-year nursing practice in a hospital or home care setting.
  • Management Experience Preferred.
  • Knowledge of medical and surgical nursing practice.
  • Ability to apply technical nursing procedure safely.
  • Ability to communicate effectively, on a verbal and written basis.
  • Ability to utilize supervision and consultation constructively.
  • Ability to effectively supervise paraprofessional staff.
  • Ability to work with individuals from differing socio-economic backgrounds.
  • Ability to apply effective teaching techniques.
  • Must have a safe driving record, as defined in URMHC Policy, and a valid NYS driver’s license.
  • Must have a telephone and reliable transportation.
  • Basic computer skills required.

Salary Range: $32.55 - $55.31
