The Clinical Nurse (CN) II provides Family-Centered Care as demonstrated by recognizing the patient, family and/or designee as a full partner in providing compassionate and coordinated care based on respect for patient’s preferences, values, and needs. Assumes active role in clinical education of patients, families and staff. Functions effectively within nursing and inter-professional teams, fostering open communication, mutual respect, and shared decision-making to achieve quality patient care. Integrates best current evidence with clinical expertise for delivery of optimal health care. Uses data to monitor the outcomes of care processes and uses improvement methods to test changes to continuously improve the quality and safety of health care systems. Minimizes risk of harm to patients and providers through both system effectiveness and individual performance. Uses information and technology to communicate, manage knowledge, mitigate error, and support decision-making.
The Breathing Institute is located on the Anschutz Campus in the Outpatient Pavilion with additional services offered across several Network of Care sites throughout the North Denver Metro, South Metro regions and Grand Junction. Our team provides comprehensive pulmonary care to pediatric patients and supports a wide range of respiratory and aerodigestive conditions through both medical management and collaborative interdisciplinary approaches.
Ambulatory pulmonary nurses work closely with an experienced team that includes pediatric pulmonologists, advanced practice providers, respiratory therapists, registered nurses, medical assistants, dietitians, and social workers. Together we coordinate care for patients with acute episodic and chronic respiratory conditions including:
As an Ambulatory Pulmonary Nurse you will be part of a highly skilled interdisciplinary team that is committed to delivering exceptional patient and family centered care. You will support care coordination across the continuum provide education and resources in the clinic setting, perform telephone triage and help guide families in managing both common and complex respiratory needs.
