Registered Nurse - RN

Role Summary:
In this role, you will be a key member of the care team, delivering personalized nursing care, coordinating with healthcare professionals, and promoting overall patient health and wellness. This is an exciting opportunity to make a meaningful difference in patients’ lives while growing your clinical expertise.

Duties / Responsibilities:

  • Provide high-quality, patient-centered nursing care, including assessment, planning, implementation, and evaluation.

  • Administer medications, treatments, and procedures according to established protocols and patient care plans.

  • Monitor patients’ conditions, document findings, and report changes to the care team promptly.

  • Educate patients and their families on disease management, health promotion, and preventive care.

  • Collaborate with the care team, physicians, and other healthcare professionals to coordinate and optimize patient care.

  • Assist in the development and implementation of individualized care plans for patients with complex needs.

  • Maintain accurate and timely patient records, ensuring compliance with all regulatory and organizational requirements.

  • Provide support for both in-person and telehealth visits, as applicable.

  • Ensure adherence to infection control, safety, and HIPAA regulations.

  • Participate in continuing education and professional development opportunities.

  • Perform other related duties as assigned.

Required Qualifications:

  • Active Registered Nurse (RN) license in CA.

  • Bachelor’s or Associate’s Degree in Nursing from an accredited program.

  • Strong clinical, assessment, and patient care skills.

  • Excellent communication, interpersonal, and patient education skills.

  • Ability and willingness to travel to patient locations, if required.

  • Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications, if applicable.

  • Valid driver’s license.

Desired Qualifications:

  • Experience in home health, community care, or outpatient nursing.

  • Ability to work collaboratively within a multidisciplinary care team.

  • Strong commitment to patient advocacy and health promotion.

  • Comfort with electronic health records and telehealth technologies.
