Overview
Join our dynamic healthcare team as a Registered Nurse (RN) – AHCCCS Rendering Provider, where your expertise will directly impact patient care and community health. Hearts in Hands CPR & Services LLC is a Phoenix-based, community-focused healthcare organization expanding its network of Registered Nurses (RNs) to provide in-home and telehealth services for patients with chronic and complex medical conditions.
We are seeking experienced, compassionate, and self-motivated Registered Nurses who are already enrolled as individual AHCCCS rendering providers to join our growing clinical team.
This role focuses on patient-centered care outside of traditional clinical settings, helping reduce hospital readmissions, improve continuity of care, and support vulnerable populations in the community.
you will deliver comprehensive nursing services to diverse populations, including individuals with complex medical needs, developmental disabilities, and those requiring specialized care such as memory care, hospice, or acute pain management. Responsibilities
Perform in-home nursing assessments and follow-up visits
- Provide telehealth monitoring and patient support
- Deliver clinical evaluations, education, and care coordination
- Document all encounters accurately and in compliance with AHCCCS standards
- Communicate effectively with providers, case managers, and interdisciplinary teams
- Identify early signs of clinical deterioration and escalate appropriately
- Support patients with chronic conditions, including respiratory-related needs..
- Utilize EMR (Electronic Medical Record) systems like Epic or Cerner to document medical records meticulously and ensure compliance with NCQA standards.
- Coordinate discharge planning, case management, and follow-up care for patients with complex conditions such as infectious diseases or trauma medicine cases.
- Engage in triage activities in urgent care or emergency room settings, prioritizing patient needs efficiently while working with individuals of all ages including pediatrics and geriatrics.
- Support infection control protocols and laboratory specimen collection/processing to maintain safety standards.
- Collaborate with multidisciplinary teams in specialty areas such as cardiology (including cardiac catheterization), ophthalmology, neurocritical care (ICU experience), or neonatal intensive care units (NICU).
- Educate patients and families on health coaching topics like medication administration, nutrition, or chronic disease management to promote long-term wellness.
- Active Registered Nurse (RN) license – State of Arizona
- Must be an active AHCCCS enrolled provider (Individual Rendering Provider)
- NPI (Type 1) required
- Current BLS certification (ACLS preferred)
- Minimum 1 year of clinical nursing experience (home health, case management, or community health preferred)
- Valid driver’s license and reliable transportation
- Ability to travel locally for home visits
Experience

- Strong in communication, documentation, and clinical judgment
- Motivated to make a real impact in the community
- Valid RN license with current registration in the state of practice.
- Demonstrated experience working within hospital medicine, critical care units (ICU/Trauma Center), emergency medicine, or post-acute settings such as nursing homes or hospice environments.
- Proficiency in using EMR/EHR systems like Epic or Cerner for documentation and patient management.
- Hands-on experience with advanced clinical procedures including IV insertion, ventilator management, ostomy care, or gastrostomy tube feeding is highly desirable.
- Background in managing acute pain cases, infectious disease care, or working with individuals with disabilities enhances your ability to excel in this role.
- Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures, and familiarity with laboratory testing enhances clinical accuracy.
- Prior experience in diverse specialties such as behavioral health, geriatrics (including MDS assessments), pediatrics (including neonatal ventilators), or occupational health is advantageous.
