Job Summary
We are seeking a licensed Registered Nurse (RN) to conduct initial client assessments prior to the start of services and perform required supervisory visits every 120 days. This role focuses on ensuring quality of care, regulatory compliance, and coordination with care teams to support client needs.
Key Responsibilities

• Conduct comprehensive nursing assessments for new clients prior to the initiation of services
• Evaluate client health status, functional needs, and appropriateness of services
• Develop and/or review care plans based on assessment findings
• Perform supervisory nursing visits at least every 120 days, or as required
• Monitor quality of care and compliance with established care plans
• Provide guidance and oversight to direct care staff as applicable
• Document all assessments, supervisory visits, and findings accurately and timely
• Communicate changes in client condition to appropriate team members
• Ensure compliance with state, federal, and organizational regulations

Qualifications

• Current, active Registered Nurse (RN) license in good standing
• Minimum of 2 years of nursing experience (home care, community health, or case management preferred)
• Strong assessment and clinical judgment skills
• Knowledge of care planning and supervisory nursing requirements
• Excellent documentation and communication skills
• Ability to work independently and manage a flexible schedule
• Valid driver’s license and reliable transportation (if field-based)

Preferred Experience

• Experience with pre-service assessments
• Supervisory or oversight nursing experience
• Familiarity with home care or community-based service models
