Job Overview
We are seeking an energetic and compassionate Registered Nurse (RN) to join our Home Care team. In this vital role, you will deliver high-quality nursing care directly in the homes of our participants, ensuring their health and safety while promoting independence. Your expertise will support a diverse caseload, including individuals with chronic conditions, memory care needs, and those requiring acute or palliative care. This position offers an exciting opportunity to make a meaningful difference in the lives of older adults by providing expert clinical assessments and personalized care.

Duties

  • Conduct comprehensive patient assessments, including vital signs, physical examinations, and medication reconciliation.
  • Develop and implement individualized care plans aligned with each participant’s needs and goals.
  • Administer medications, injections, IV infusions, and perform wound suturing as needed.
  • Manage complex conditions such as dementia, Alzheimer's disease, chronic pain, and post-acute recovery.
  • Coordinate with interdisciplinary teams utilizing EMR (Electronic Medical Records) systems like Epic or Cerner for accurate documentation.
  • Provide education on disease management, medication adherence, nutrition, and health coaching to patients and caregivers.
  • Perform specialized procedures including catheterization, ostomy care, tube feeding management, and sterile techniques for infection control.

Requirements

  • Valid RN license in the state of practice with current CPR/BLS certification.
  • Critical care experience in settings such as ICU or emergency medicine is highly preferred.
  • Experience working with individuals with developmental disabilities or in memory/cognitive impairment settings.
  • Proficiency with EMR/EHR systems such as Epic or Cerner; familiarity with ICD coding (ICD-9/10) is a plus.
  • Knowledge of infection control practices, aseptic techniques, and laboratory procedures.
  • Ability to perform physical examinations, venipuncture, phlebotomy, and specimen collection accurately.
  • Strong communication skills for triage assessments and patient education.
  • Experience in home & community care environments; familiarity with long-term care or hospice is advantageous.
