Registered Nurse - Telehealth

Telehealth Nurses serve as a key clinical liaison between patients and providers in a busy pulmonary outpatient practice. This role provides telephone triage, patient education, care coordination, and clinical support to ensure safe, timely, and effective patient care. The Phone Nurse assesses symptoms, prioritizes patient needs, and collaborates closely with physicians, advanced practice providers, and team members.

*** This is NOT a remote position. Candidate would work from Administrative Office on-site ***

ESSENTIAL RESPONSIBILITIES

• Perform telephone triage for pulmonary and sleep medicine patients using evidence based protocols
• Conduct patient assessments via phone to evaluate symptoms and determine appropriate care
• Determine urgency of care needs and escalate appropriately to providers
• Document all patient interactions accurately and timely within the electronic medical record
• Communicate effectively with providers and team members through the electronic medical record and other digital platforms
• Collaborate staff to support continuity of care
• Maintain HIPAA compliance and patient confidentiality at all times

KNOWLEDGE, SKILLS, AND ABILITIES

• Understanding of pulmonary disease processes and respiratory medications
• Familiarity with oxygen therapy, inhalers, nebulizers, CPAP and BiPAP devices a plus
• Ability to recognize red flag symptoms requiring urgent intervention
• Professional demeanor with strong patient service orientation
• Ability to work both independently and collaboratively within a team
• Strong clinical assessment and critical thinking skills
• Excellent verbal communication skills with ability to explain complex medical information clearly
• Must be able to communicate effectively and professionally through electronic platforms including EMR messaging, task management systems, and secure email
• Experience with electronic medical records
• Ability to manage high call volumes in a fast paced environment
• Strong organizational and time management skills

QUALIFICATIONS

• Active Registered Nurse license in the state of practice
• Minimum of 2 years of clinical nursing experience required
• Experience in pulmonary, critical care, internal medicine, or triage nursing preferred

Experience: At least 2 years clinical nursing experience

Ceritification or License(s): Current Registered Nurse (RN)
