Requirements:

• Current Registered Physical Therapy License in the State of Michigan
• Minimum of 1-2 years of home care experience
• Make home visits for patient population in the Metro Detroit area

• Valid driver's license with reliable insured transportation
• Knowledge of OASIS and Medicare documentation requirements
• Computer proficiency
• Positive attitude for professional growth
