Under the general supervision of the Director of Rehab, responsible for the management of day-to-day operations, department personnel, and patient care for specific patient populations who require the needs of rehabilitation services. As a member of the Home Care Rehab management team, assists in the development of Home Care Rehabilitation department goals, objectives and standards, quality assurance, department education, and budget development, preparation and control.
Coordinates department activities to include regulatory requirements, equipment quality control / maintenance, and performance improvement. Responsible for hiring, supervising and evaluating team members. Serves as an effective liaison between department staff, leaders, and other department leaders and medical staff.


  • Work collaboratively with Director on preparation of annual budget, and expense management. Control Operating expenses in compliance with budget. Provide continuous analysis and reports on variations from established budgets. Has accountability in budget variance reporting and expenses
  • Work collaboratively with Director on strategic planning to support organizational goal alignment.
  • Plan, implement, and communicate Rehab department policies and procedures utilizing operating model. Work with Director to ensure regulatory compliance is maintained. Ensure compliance for policies and procedures. Work with quality stakeholders to ensure safety standards are being met and risks are being minimized. Maintain accountability for compliance with all regulatory standards as well as compliance with all guidelines and regulations.
  • Maintains accountability for compliance with all regulatory standards including local, state, and federal laws, as well as The Joint Commission, Connecticut Department of Public Health, Connecticut licensing requirements and organizational quality improvement programs.
  • Manage the operational activities of Rehab department and/or locations including management of department staff. Manage staff relations including performance management, staff satisfaction, conflict management, etc. Oversee scheduling, recruitment, and student engagements. Provide developmental opportunities for employees through a comprehensive orientation program, in-service programs, clear performance standards and appropriate feedback through the performance appraisal process. Oversight of HealthStream.
  • Assess quality of patient care. Participate in frequent home/joint visits.
  • Delegate patient care tasks. Provide clinical supervision in area of specialty.
  • Manage efficiency and visit volume; assess visit utilization and timeliness to care. Daily management of work Ques.
  • Participation in regional committees and projects including but not limited to:
    • Safety Task Force
    • Interdisciplinary Team Meetings
    • Transition of Care collaboration with system partners
  • May provide direct Patient Care as needed: Work as a Physical Therapist, Occupational Therapist, or Speech Language Pathologist adhering to all responsibilities and requirements detailed in Physical Therapist, Occupational Therapist, or Speech Language Pathologist job description.
  • Perform other related duties as required

Qualifications


Education: Bachelors of Science Degree in Physical Therapy or Bachelors of Science Degree in Occupational Therapy or Master’s Degree in Speech Language Pathology from an accredited Speech Pathology program required. Master’s Degree in Physical Therapy or Doctor of Physical Therapy or Master’s Degree in Occupational Therapy or Doctor of Occupational Therapy or Clinical Doctorate or PhD in Speech Pathology preferred.


Experience: 3 Years experience as a supervisor or manager required. 5 Years’ experience as a manager preferred.

Licensure, Certification, Registration
All: Basic Life Support required.
Physical Therapist: Current State of CT Physical Therapy license required.
Occupational Therapist: Current State of CT Occupational Therapy license required.
Speech Language Pathologist: Current State of Connecticut license in Speech Language Pathology. Certificate of Clinical Competence with passing score on the national SLP Praxis® exam required.

Knowledge, Skills and Ability Requirements
  • Understanding of the business aspect of rehabilitation.
  • Ability to analyze and develop key financial and operational metrics.
  • Computer proficiency in Microsoft Word, Excel, PowerPoint as well as electronic heath records and other software applications.
  • Provide two-way communication between staff and HHCRN leadership.
  • Practices, promotes, and models HHC Leadership Behaviors and Lean Daily Management.
  • Create and support high performing teams and eliminate barriers for area of control.
  • Fosters an environment that encourages creativity and new ideas for continuous improvement and reduce unnecessary variation and implement standard work and best practice.
