Job Title: Care Manager
Location: Remote (Must reside in NY | Quarterly in-person meetings required)
Salary: $50,000 – $60,000
Job Type: Full-Time
We are seeking compassionate and organized Care Managers to provide person-centered care coordination services to individuals within the OPWDD system. This role focuses on delivering the six core Health Home Care Management services while supporting members and families through advocacy, care coordination, referrals, and ongoing engagement.
This is a primarily remote position with quarterly in-person meetings in New York.
Comprehensive Care Management
Conduct comprehensive assessments identifying medical, behavioral, developmental, and social service needs
Develop and maintain individualized Life Plans with members, families, providers, and support systems
Manage a caseload of approximately 35–40 members
Conduct required face-to-face visits (monthly, quarterly, or bi-annually depending on member needs and regulations)
Care Coordination & Health Promotion
Coordinate medical, behavioral health, and social service supports
Monitor treatment adherence and update Life Plans as needed
Facilitate access to appointments and community resources
Collaborate with interdisciplinary teams and clinical staff
Support post-hospital and rehabilitation transitions
Transitional Care
Assist members during transitions between levels of care and crisis situations
Utilize Health Information Technology (HIT) systems to coordinate services and communication
Individual & Family Support
Maintain regular communication and engagement with members and families
Ensure care plans reflect member goals and preferences
Connect families with peer support and community resources
Referral & Community Support Services
Identify and coordinate referrals to community-based programs and services
Monitor follow-up and engagement with providers and supports
Documentation & Compliance
Maintain accurate records in the Electronic Health Record system
Ensure compliance with HIPAA and OPWDD regulations
Assist members with maintaining Medicaid, SSI, SNAP, and other benefits
Candidates must meet one of the following:
Bachelor’s Degree + 2 years of relevant experience
OR
RN License + 2 years of relevant experience
OR
Master’s Degree + 1 year of relevant experience
MSC Service Coordinators employed prior to July 1, 2018 may qualify under grandfathering provisions.
Strong interpersonal and communication skills
Ability to manage conflict and de-escalate situations
Excellent organizational and time management abilities
Strong Microsoft Office and computer skills
Ability to prioritize and manage multiple responsibilities
Experience in care coordination, OPWDD, case management, healthcare, or social services preferred
