Remote Certified Medical Assistant (CMA) – CCM & APCM Coordinator

Job Overview

We are looking for a motivated and patient-focused Certified Medical Assistant (CMA) to help us launch and enroll patients in our new Chronic Care Management (CCM) and Advanced Primary Care Management (APCM) programs.

This is a remote position that requires a proactive professional who enjoys connecting with patients. You will be part of the first wave of practices implementing the innovative 2026 APCM model, directly impacting how we provide longitudinal care to our community.

Key Responsibilities

  • Program Enrollment: Proactively call eligible patients to explain how CCM/APCM supports their health and document their formal verbal consent.
  • Care Coordination: Conduct monthly telephonic check-ins to monitor health status, medication adherence, and progress toward care goals.
  • Documentation: Maintain clear and accurate records in the EHR, ensuring all Medicare-required elements are met for monthly billing.
  • Patient Support: Act as a dedicated bridge between the patient and the physician/NP to coordinate refills, specialist care, and follow-ups.

Requirements

  • Current CMA (AAMA) or RMA (AMT) certification is mandatory.
  • High level of motivation to reach outreach goals and build the patient panel.
  • Exceptional phone presence—must be empathetic, clear, and professional.
  • Ability to work independently from a quiet, HIPAA-compliant home office.
  • Proficiency with Electronic Health Records (EHR).

Pay: $25.00 - $27.00 per hour
