Remote RN Care Manager

RN Care Manager (Remote) - Applicants MUST reside in Connecticut to be considered

  • Monday thru Friday 9:30am to 6pm
  • Remote position plus 3-5 "hands-off" visits (virtual or in-person) per week

About the position:

The RN Care Manager communicates with the clinical team, provider network, and the community to create a process of assessment, planning and facilitation to meet the member’s heath care needs across the continuum of care. The RN assists the member by functioning as an advocate for options and services by communicating effectively with other members of the healthcare team and by utilizing available resources to promote quality cost effective outcomes and member autonomy.

  • Develops, monitors and evaluates the effectiveness of the care plan and modifies as necessary.
  • Provides education and coordination of outpatient care to members selected for Case Management.
  • Identifies and refers quality of care, coordination of benefits and third party liability issues as appropriate.
  • Orients new staff as requested.
  • Engages with members through phone, video conferencing and face-to-face outreach; meets with providers and community organizations to coordinate care.
  • Conducts assessments to identify the member’s needs and develops a specific care plan to address objectives, barriers and behaviorally specific goals identified during the assessment.

What we’re looking for:

  • Must reside in Connecticut
  • Connecticut RN License required
  • Minimum 2 years of RN experience is required
  • Strong knowledge of care plans and care plan development/modification is a must
  • Home Health experience is strongly preferred
  • Experience making home and provider visits is preferred
  • Experience working with the behavioral health population is preferred
