The RN Case Manager is a key member of our Medication-Assisted Treatment (MAT) team, responsible for supporting treatment retention and providing patient-centered care for individuals with substance use disorders in an integrated primary care setting. As a member of the MAT Team (which includes MAT leadership, medical assistants, licensed mental health and medical providers), they will participate in the case management of MAT patients, including those who may need complex care coordination across multiple life domains and disciplines (medical, behavioral health, hospital in-patient, and ER visits). This also includes operational support to ensure that communication is functioning smoothly related to scheduling and treatment coordination of MAT patients.
This role combines clinical nursing expertise with case management to ensure safe, effective MAT services and continuity of care across medical, behavioral health, and community resources.
As part of a progressive care model, this position will evolve into a Behavioral Health Integration (BHI) and/or Collaborative Care Model (CoCM) role, serving as a bridge between primary care, behavioral health, and psychiatric services.
Essential Duties and Responsibilities / MAT Program Responsibilities:
- In collaboration with the patient, behavioral health and medical providers, assists with meeting the needs of MAT patients, including education, care coordination, follow-up care, and outside referrals. This may involve face-to-face, phone meetings, and review of electronic messaging

Provide direct and indirect nursing care for MAT patients, including patient education, monitoring, and medication adherence support.

Conduct intake assessments and periodic clinical reviews, including vital signs and mental health screenings.

Collaborate with medical and behavioral health providers to create and update care plans.

Support harm reduction strategies, overdose prevention education, and safe medication use.

Coordinate with pharmacies to ensure timely access to MAT medications.

Identify gaps in communication and operational workflows and recommend changes as needed

Track and support patient participation in MAT, behavioral health and primary care services, use motivational interviewing and shared decision making to identify barriers and when feasible to address full participation in treatment.

Participate in weekly MAT case conferences and interdisciplinary huddles to provide contextual patient information (e.g., drug use history, strengths, stressors).

Review hospital, inpatient, and ER reports for MAT patients; facilitate follow-up appointments post-discharge.

Act as a liaison with local hospitals and community agencies to improve continuity of care.

Maintain compliance with confidentiality laws and organizational policies.

Gather and track program data for quality improvement initiatives.

Attend warm handoffs and assist with group visits or patient support sessions when indicated.

Transition to BHI/CoCM Responsibilities:

Serve as Behavioral Health Care Manager within a collaborative team (PCP, psychiatric consultant, behavioral health providers).

Monitor progress using validated tools (PHQ-9, GAD-7, AUDIT, DAST).

Deliver evidence-based brief interventions (behavioral activation, problem-solving therapy, motivational interviewing).
-
Coordinate psychiatric recommendations into patient care plans.

Document and report per BHI/CoCM billing requirements.

Bachelor’s degree in nursing, social work, behavioral health, or related field

Active Registered Nurse (RN) license in TN if an RN.
- Minimum 2 years of clinical experience, preferably in psychiatry, mental health, or substance use treatment.

Knowledge of addiction medicine principles and trauma-informed care.

Strong care coordination, communication, and patient engagement skills.

Ability to work within a team-based, integrated care environment.

Preferred:

Experience in community health or FQHC setting.

Familiarity with MAT

Bilingual (English/Spanish) strongly preferred.
