RN Case Manager Hospice (North County)

Hospice Care – Routine, Admissions, Supervisory & Recertification Visits

We are a compassionate and dependable hospice agency seeking a Registered Nurse to support our growing team in North County SD. This role is ideal for an independent, experienced RN who is comfortable working autonomously and driving to patient homes and facilities throughout the region.

We are looking for someone to assist with routine nursing visits, supervisory visits, recertification assessments, and new admissions as needed. The position is per diem (as needed), offering flexibility to fit your schedule while still making a powerful impact in the lives of patients and families during their most important moments.

Essential Responsibilities:

  • Perform routine, supervisory, recertification, and admission nursing visits as assigned
  • Conduct in-home and facility assessments to evaluate patient status and needs
  • Develop individualized care plans and provide education to patients and caregivers
  • Administer medications and manage symptoms in accordance with hospice protocols
  • Communicate and coordinate care with the interdisciplinary team and referral partners
  • Complete timely, accurate documentation in the EMR

Qualifications:

  • Current California RN license
  • Current CPR certification (AHA BLS-compliant)
  • Valid driver’s license and auto insurance
  • Reliable transportation and willingness to drive throughout the county
  • Ability to lift and assist patients as needed

Pay: $57.00 - $60.00 per hour
