Additional Information: 
Licensed master&rsquo;s level social worker in state of AZ OR RN State of Arizona, At least five years of Case Management experience in an acute care hospital, Assist with Initial Assessments, agreeability and DC plan with post-acute preferences. Heavy in DC planning experience
