RN CLINICAL COORDINATOR
Responsibilities:
-This is an 8-5 Mon-Fri position in the office.
- Coordinates the delivery of patient care services of all disciplines in accordance with the POC, to ensure goal-directed and cost-efficient home care for patients.
- Promotes effective communication between patient/family, team members and other health care professionals. Communicates promptly and professionally with care coordinators in the office regarding any changes in patient status.
- Writes up OASIS reports (SOC, ROC, REC) and verifies accuracy of information when reports are called in by field staff.
- Receives/documents/routes labs, orders, and prescriptions to facilities, field staff, scheduler and medical records.
- Provides guidance to clinicians on documentation.
- Orders and tracks delivery of DME.
- Ensures appropriate supplies are ordered and dispensed for the patient in a timely manner.
- Reviews notes in QA to ensure that there is a skilled need on each note; distributes notes to patient charts.
- Reviews patient charts prior to case conference (including review of orders, frequencies, communication notes, vital signs outside of normal parameters).
- Documents pertinent clinical information provided by other caregivers in patient charts via communication notes.
- Identifies and suggests ideas for performance improvement.
- Periodically administers skilled care to patients; participates in on call rotation when needed.
Knowledge, Skills, and Abilities:
- Graduate of an accredited school of nursing; must be licensed by the Texas State Board of Nurse Examiners.
- One (1) year home health experience. Please do not apply if you do not have at least one year of home health experience.
- Knowledge of home healthcare regulatory requirements and accreditation standards.
- Minimum two (2) years experience as a registered nurse in a clinical practice.
- Highly organized; detail-oriented; able to prioritize.
- Must possess excellent interpersonal skills and be professional in appearance and conduct.
Pay: $75,000.00 - $85,000.00 per year
