RN Clinical Support & Medication Safety (

Overview

Join our growing healthcare team as an RN Clinical Support & Medication Safety Specialist, where your expertise will help strengthen clinical quality and medication safety across community-based programs serving adults with developmental disabilities. This role offers an opportunity to step out of bedside nursing and into a systems-driven environment where your clinical judgment, organization, and attention to detail directly improve outcomes across multiple settings.

You will support medication safety, clinical coordination, and health-risk monitoring across Day Support, Group Homes, and In-Home services. Working alongside operations and leadership, you will help ensure that care is consistent, safe, and aligned with state regulations while contributing to a culture of accountability and continuous improvement.

Responsibilities

  • Provide clinical support across programs by reviewing medication administration practices, MAR accuracy, and staff adherence to protocols
  • Monitor medication safety systems including storage, administration, documentation, and error follow-up
  • Support medication-certified staff through coaching, observation, and reinforcement of best practices
  • Coordinate with pharmacies and providers to ensure timely refills, clear orders, and accurate medication changes
  • Review hospital discharges and ensure safe transitions back into services (med reconciliation, follow-ups, restrictions)
  • Track and support completion of medical appointments, ensuring outcomes are documented and follow-ups are completed
  • Identify trends in client health (falls, seizures, respiratory issues, behavioral changes) and escalate concerns appropriately
  • Assist with incident review related to medication and health issues, helping identify patterns and prevention strategies
  • Provide targeted clinical training based on real issues (medication errors, health observation, documentation gaps)
  • Ensure compliance with HIPAA and maintain accurate, timely documentation across systems
  • Collaborate with operations, program leaders, and compliance staff to maintain visibility into client health and safety

Experience

  • Active RN license in Virginia (required)
  • Experience in behavioral health, developmental disabilities (DD/IDD), or community-based services preferred
  • Strong understanding of medication administration, MAR systems, and clinical documentation
  • Experience coordinating care with providers, pharmacies, or multidisciplinary teams
  • Comfortable working in a structured, non-bedside environment focused on systems and oversight
  • Strong communication, organization, and critical thinking skills
  • Ability to manage multiple clients/programs while maintaining attention to detail
  • Experience with EHR/EMR systems (Therap or similar platforms preferred, but not required)
  • Ability to identify early warning signs in client health and escalate appropriately
  • Valid driver’s license and reliable transportation required

Additional Information

  • Schedule: Monday–Friday, 9:00 AM – 5:00 PM (emergencies only outside standard hours)
  • Work Setting: Community-based (Group Homes, Day Support, In-Home) — not hospital-based
  • This role does not involve routine bedside assignments or shift coverage

Job Type: Full-time

Pay: $80,000.00 - $90,000.00 per year
