Position Summary



The Aultman Registered Nurse (RN) is a professional nurse that provides direct, primary nursing care to patients/residents. The RN delegates and coordinates patient care, based upon a documented assessment of the patient’s/resident’s needs. The RN supervises the care provided by individuals such as licensed practical nurses, patient care technicians/state tested nursing assistants/nurse aides, medical assistants, etc., if applicable.




Department Summary



The RN Care Coordinator is an integral part of the cancer team, responsible for guiding patients through cancer treatment, providing support, and triaging ongoing patient needs. Duties will vary according to patient population, disease state, workflow needs, but the overall goal is to provide seamless care and a point of contact for healthcare providers, patients, and family members. Inherent in this job is the ability to communicate with health care providers and act as a liaison to the patient and family.




Qualifications



  • Graduate of an accredited school of nursing; BSN preferred.


  • Current, active licensure as a Registered Nurse through the Ohio Board of Nursing.


  • Certification preferred for RNs meeting applicable certification eligibility requirements.


  • Membership in professional nursing organization preferred.


  • Obtain and maintain current American Heart Association (AHA) training in Basic Life Support (BLS) utilizing Resuscitation Quality Improvement (RQI) is required




Department Qualifications



  • Bachelor of Science in Nursing recommended by not required


  • Oncology Nursing Certification preferred with a minimum of 2 years experience in oncology nursing


  • Displays an attitude of respect and excellent customer service

Responsibilities & Expectations



  • RN applies the nursing process of assessment, planning interventions, accurately implements and/or delegates patient care, evaluates that care, and accurately communicates patient information in a timely manner.


  • RN is a patient/resident advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort, and satisfaction.


  • Attains knowledge and competency that reflects current nursing practice.


  • Demonstrates quality and effectiveness of nursing practice.


  • Adheres to professional standards and guidelines, including ethical actions, relevant statutes, rules, and regulations, during nursing practice.


  • Collaborates with patient, family, and others in the conduct of nursing practice.


  • Exhibits High Reliability Organization (HRO) tools and tones.


  • Provides leadership in the professional practice setting and contributes to the professional development of peers, colleagues, and students.


  • Integrates research findings into practice.


  • Responsible for timely review of emails, department communications, and applicable learning assignments and completion as applicable.




Department Responsibilities & Expectations



  • Each RN Care Coordinator will work directly with an assigned physician


  • Provides patient education related to chemotherapy treatment and management of side effects


  • Obtains signature on chemotherapy consent for physicians, providing a copy to the Infusion Center for Cerner


  • Enters staging of disease in EMR


  • Loads chemotherapy regimen as prescribed by physicians, including any changes in treatment


  • Provides daily orders to the Infusion Center from assigned physician following physician review, and approval w/ signatures


  • Arranges supportive meds for home: antiemetics and/or steroids


  • Contacts patient day after first treatment to mitigate issues and provide support


  • Provides symptom management for patients on phone or walk-in clinic


  • Provides phone triage support, appropriately prioritizes calls and patient needs


  • Collaborates with physicians and nurse practitioners for symptom management issues


  • Coordinates concurrent chemotherapy and radiation as ordered


  • Assists physicians with special requests


  • Delegates tasks to phone triage staff as needed


  • Utilizes resources in Cancer Program to assist in care: Navigation, Social Worker, and Research Coordinators


  • Networks with appropriate agencies for referrals, including outreach services and other funding associations


  • Maintains an open line of communication at the discretion of the patient throughout the continuum of care


  • Keeps accurate and timely records in the patient medical record


  • Demonstrates lifelong learning through a knowledge and understanding of best practice and evidence-based guidelines for cancer treatment and screening recommendations


  • Creates and fosters a work environment that is consistent with the Mission, Vision and Values of the Aultman Health Foundation


  • Supports strategic organizational goals


  • Abides by the Aultman Code of Conduct at all times


  • Other responsibilities as assigned by the Director



Working Conditions



  • Meal and break times are coordinated with other staff members to maintain adequate coverage.


  • Shifts are scheduled or changed due to the staffing needs of the unit/department based upon volume and acuity.


  • Work weekends and holidays as assigned or as applicable based on department needs.


  • Cross training depending upon department needs.
